“瞄准器”状皮肤切开缝合法与传统一期缝合法处理肠造口还纳术后腹壁切口的疗效对比

摘要

目的 观察“瞄准器”状皮肤切开缝合技术与传统的一期缝合法处理肠造口还纳术后腹壁切口的效果.方法 回顾性分析2012年3月至2014年9月间本科室收治的65例肠造口还纳术患者资料,按照造口还纳后腹壁皮肤切口缝合处理的不同,将研究对象分为对照组(n=32)及观察组(n=33),对照组采用传统的一期缝合处理造口还纳后腹壁皮肤切口,观察组采用“瞄准器”状皮肤切开缝合法.比较两组患者的手术时间、手术出血量、切口感染率、住院时间、切口疼痛及治疗满意度.结果 两组病人手术均顺利完成.观察组患者伤口感染率及术后疼痛评分均显著低于对照组,治疗满意度评分显著高于对照组(均P＜ 0.05).两组手术出血量、手术时间、住院时间差异均无统计学意义(均P＞ 0.05).结论 与传统的一期缝合法相比,“瞄准器”状皮肤切开缝合技术处理腹壁皮肤切口可有效降低肠造口还纳术后的伤口感染率及疼痛感,患者治疗满意度更高.%Objective To compare the clinical effect of ‘Gunsight’ skin incision and suture technique with traditional primary suture in the treatment of intestinal colostomy closure after abdominal wall incision.Methods 65 patients of colostomy from March 2012 to September 2014 in our hospital were also retrospectively analzed,the subjects were divided into control group (n =32) and observation group (n =33).The patients in the control group were treated with suture technique with traditional primary suture while the observation group with the ‘ sight’ shaped skin incision suture.The operation time,bleeding volume,incision infection rate,length of stay,incision pain and satisfaction were compared between the two groups.Results The operation was completed successfully in the two groups.The wound infection rate and postoperative pain score of the observation group were significantly lower than those of the control group,and the satisfaction score of the treatment group was obviously higher than that of the control group (P ＜ 0.05).There were no significant difference in the amount of bleeding,the operation time and the length of stay between the two groups (P ＞ 0.05).Conclusion Compared with the traditional primary suture,the ‘sight’ shaped skin incision suture technique for treatment of abdominal wall incision can effectively reduce intestinal colostomy closure after wound infection and pain,which has a higher treatment satisfaction.
