血清锁链素、克拉拉细胞蛋白-16水平与慢性阻塞性肺疾病严重程度关系

Objective To explore the correlation of plasma desmosine and Clara cell protein-16 ( CCP-16 ) levels with the severity of chronic obstructive pulmonary disease. Methods 100 COPD patients (66 cases of stable COPD and 34 cases of AECOPD) and 33 control subjects were recruited from January 2014 to December 2015. The COPD patients were divided into the high risk group (n=43) and the low risk group (n=23) according to the crite-ria of Global Initiative for Chronic Obstructive Lung Disease ( GOLD) strategy. The plasma concentrations of desmo-sine and CCP-16 were measured by ELISA. Results The plasma concentration of desmosine and CCP-16 were sig-nificantly lower in AECOPD patients than in stable COPD patients and the control group (P<0. 05), and they were obviously lower in the high risk group than in the low risk group (P<0. 05). The plasma concentration of desmosine and CCP-16 were positively correlated with FEV1 (r= -0. 584, P=0. 002; r= -0. 446, P=0. 012), and nega-tively correlated with mMRC (r = -0. 583, P <0. 001; r = -0. 419, P =0. 002), SGRQ (r = -0. 548, P =0. 001;r=0. 550, P=0. 003) of the stable COPD patients. Conclusion Plasma desmosine andCCP-16 levels can effectively evaluate the severity of COPD patients, and it could be useful for the diagnosis of AECOPD and severity assessment of stable COPD.%目的：探讨血清锁链素、克拉拉细胞蛋白-16水平与慢性阻塞性肺疾病(慢阻肺)病情严重程度的关系。方法选取2014年1月至2015年12月于我院就诊的慢阻肺患者100例(其中稳定期组66例,急性加重期组34例),并以33例同期健康体检者为对照组。稳定期慢阻肺组根据慢阻肺全球倡议( GOLD)分为高风险组(n=43)和低风险组(n=23)。应用酶联免疫吸附(ELISA)法检测全部受试者入组当天血清锁链素、克拉拉细胞蛋白-16水平并进行统计分析。结果慢阻肺急性加重期组血清锁链素、克拉拉细胞蛋白-16水平显著低于慢阻肺稳定期组及健康对照组。慢阻肺高风险组血清锁链素、克拉拉细胞蛋白-16水平显著低于慢阻肺低风险组。血清锁链素、克拉拉细胞蛋白-16水平分别与慢阻肺稳定期患者FEV1( r=-0．584,P=0．002；r=-0．446,P=0．012)呈正相关,而与mMRC评分(r=-0．583,P<0．001；r=-0．419,P=0．002)、SGRQ评分(r=-0．548, P=0．001；r=0．550,P=0．003)之间均呈负相关关系。结论血清锁链素、克拉拉细胞蛋白-16与慢阻肺患者病情的严重程度明显相关,其可作为判断慢阻肺急性加重及稳定期严重程度分级的指标。
