Objective To observe the effect of Doula delivery combined with AIDET communication mode on mental state and delivery outcome of elderly parturient women .Methods 120 elderly parturient w omen w ere divided into tw o g roups according to the random number table method ,60 cases in each group .On the basis of traditional Doula analgesia delivery ,the observation g roup adopt AIDET co m m unication mode to guide maternal delivery .T he control g roup w as accompanied by traditional Doula analgesia delivery . Anxiety self-rating scale w as used to assess anxiety status ,and V AS w as used to assess pain degree .T he outcomes of the two groups were counted ,including the mode of delivery and the health status of the new- born (assessed using the Apgar score ).Satisfaction survey w as conducted using the self-made maternity care satisfaction questionnaire .Results T here w as no significant difference in the scores of anxiety self- rating scale betw een the tw o groups before delivery ( P＞ 0 .05 ).T here w ere significant differences be- tw een the tw o groups at 3 cm in the cervix and 2 h after delivery ( P＜0 .05).T here w ere significant differ- ences in visual analogue pain scores betw een the tw o groups at different stages of delivery ( P＜0 .05 or 0 .01).T he natural delivery rate of the observation g roup w as higher than that of the control g roup ,and the perineal side cut and cesarean section rate w ere low er than the control group ( P＜0 .05).T here w as no significant difference in Apgar scores betw een the tw o groups at 1 min ,5 min , and 10 min af ter birth ( P＞0 .05).T he satisfaction score of the observation group w as significantly higher than that of the con- trol group ( P＜0 .01).Conclusion AIDET com m unication mode combined w ith Doula delivery can allevi- ate the anxiety of older w omen ,reduce the painful feeling during childbirth ,promote natural childbirth and increase their satisfaction .%目的 探讨导乐分娩联合AIDET沟通模式对高龄产妇心理状态与分娩结局的影响.方法 将120例高龄产妇按随机数字表法分为两组,每组各60例.观察组在常规导乐镇痛陪伴分娩的基础上,采用AIDET沟通模式指导产妇分娩;对照组采用常规导乐镇痛陪伴式分娩.采用焦虑自评量表评定焦虑状况、视觉模拟疼痛评分法评定疼痛程度;统计两组的分娩结局,包括分娩方式及新生儿健康状况(采用Apgar评分法评估) ;采用自拟产科护理满意度调查表进行满意度调查.结果 分娩前两组焦虑自评量表评分比较差异无统计学意义(P＞0 .05) ,宫口开3 cm及分娩后2 h两组比较差异均有统计学意义(P＜0 .05).不同分娩阶段,两组视觉模拟疼痛评分法评分比较差异均有统计学意义(P＜0 .05或0 .01) .观察组自然分娩率高于对照组,会阴侧切、剖宫产率低于对照组(P＜0 .05) ;两组新生儿出生后1 min 、5 min 、10 min的Apgar评分比较差异无统计学意义(P＞0 .05).观察组满意度评分显著高于对照组(P＜0 .01).结论 AIDET沟通模式联合导乐镇痛陪伴分娩可缓解高龄产妇的焦虑情绪,降低分娩时痛觉感受,促进自然分娩,提高其满意度.
