Objective To compare the curative effect of anatomical locking plate and hook plate in the treatment of distal clavicle fracture of Neer type Ⅱ. Methods Anatomical locking plate fixation (18 cases) and hook plate fixa-tion (14 cases) were used in 32 patients with distal clavicle fracture of Neer typeⅡ. Results All patients were fol-lowed up for 9~20 months. All patients′fractures were healed without wound infection, internal fixation failure and other complications. Two cases of patients with shoulder pain and limitation of shoulder abduction and 3 cases with subacromial bone erosion occurred in the hook plate group, postoperative fixator removed within 6~8 months, shoul-der joint pain gradually relieved and function recovered well. The shoulder joint function of anatomical locking plate group was better than that of the hook plate group in 3,6 months after operation(P<0. 01). Three months after im-plant removal, there was no statistically significant difference in shoulder joint function between two groups ( P >0. 05). Conclusions Anatomical locking plate and the hook plate are effective methods for treatment of distal clavi-cle fracture of Neer typeⅡ, we should choose suitable internal fixation according to different fracture.%目的：比较解剖锁定板和钩钢板治疗锁骨远端NearⅡ型骨折的疗效。方法对32例锁骨远端NeerⅡ型骨折患者分别采用解剖锁定板固定(18例)和钩钢板固定(14例)。结果患者均获得随访,时间9~20个月。患者骨折均愈合,无伤口感染、内固定失效等并发症发生。钩钢板组出现2例肩关节疼痛、外展受限,3例肩峰下骨侵蚀,于术后6~8个月取出内固定物,肩关节疼痛逐渐缓解,功能恢复良好。术后3、6个月解剖锁定板组肩关节功能优于钩钢板组(P<0．01),内固定物取出后3个月,两组肩关节功能比较差异无统计学意义(P>0．05)。结论解剖锁定板和钩钢板都是治疗锁骨远端NeerⅡ型骨折的有效方法,应根据不同的骨折选择合适的内固定物。
