右美托咪定联合舒芬太尼在手指再植术患者自控镇痛中的应用效果评价

摘要

Objective To study the value of dexmedetomidine combined with sufentanil in self-controlled analgesia in finger replantation patients.Methods A total of 120 patients who underwent finger replantation in our hospital were selected as subjects.All the patients were divided into two groups according to the random number table method,with 60 cases in each group.Patients of both groups received brachial plexus block anesthesia.The control group and the observation group were treated with analgesic pump with 2 μg/kg sufentanil in 100 mL normal saline,and 2 μg/kg sufentanil as well as 200 μg dexmedetomidine,respectively.The self-control analgesic efficacy and effects of sedation,and adverse events were compared and analyzed in both groups.Results The postoperative VAS score of the observation group was lower than that of the control group at 4,12 and 24 h after operation (P ＜ 0.05).The sedative effect of the observation group was significantly better than that of the control group at 4,12 and 24 h after operation (P ＜0.05);but there was no significant difference in the total incidence of adverse reactions between the two groups (P ＞ 0.05).Conclusion For patients with finger replantation,dexmedetomidine combined with sufentanil can effectively improve the effect of controlled analgesia and sedative effect,and reduce the incidence of adverse reactions,so it is worthy of clinical promotion.%目的 探讨右美托咪定联合舒芬太尼在手指再植术患者自控镇痛中应用价值.方法 选取接受手指再植术治疗的120例患者,根据随机数表法分为对照组、观察组各60例.2组患者均接受高频超声引导臂丛神经阻滞麻醉,对照组采用镇痛泵100 mL生理盐水内加入2μg/kg舒芬太尼镇痛,观察组采用镇痛泵100 mL生理盐水内加入2μg/kg舒芬太尼、200 μg右美托咪定镇痛.比较2组患者自控镇痛效果、镇静效果及不良反应发生率.结果 观察组患者术后4、12、24 h的VAS评分均显著低于对照组(P＜0.05);观察组患者术后4、12、24 h的镇静效果均显著优于对照组(P＜0.05);2组患者不良反应总发生率比较,差异无统计学意义(P＞0.05).结论 对手指再植术患者而言,右美托咪定联合舒芬太尼能有效提高自控镇痛效果、镇静效果,减少不良反应发生.
