围产期窒息对新生儿心肌酶学和肾功能的影响

         

摘要

Objective To evaluate the influence of perinatal asphyxia on cardiac enzymes and renal function in newborns. Methods Renal function and cardiac enzymes including SGOT, LDH, CK and CK - MB were investigated in 90 newborns with perinatal asphyxia. Results CK and CK - MB in cord blood and peripheral blood of the newborns with perinatal asphyxia were much higher than those in normal neonates ( P <0.05 or P < 0.01 ). BUN and creatinine were also increased in the peripheral blood of the newborns with perinatal asphyxia than those in normal neonates ( P < 0.05 or P < 0.01 ) and the impairment of renal function was positively correlated with the severity of the asphyxia. In addition, the impairment of renal function was easy to occur in preterm neonates than that in term neonates ( P < 0.05 ). Conclusion Cardiac enzymes and renal function should be examined in neonates with perinatal asphyxia within 24 hours after they were born. It is important for the early diagnosis and treatment and prognosis.%目的 探讨围产期窒息对新生儿心肌酶学和肾功能的影响.方法 对90例围产期窒息患儿进行脐动脉血心肌酶谱、新生儿外周血心肌酶谱和肾功能进行测定.结果 围产期窒息患儿脐血和外周血肌酸激酶(CK)和肌酸激酶同工酶(CK-MB)均较正常新生儿的显著升高(P<0.05),且外周血的升高更明显(P<0.01).围产期窒息患儿外周血尿素氮和肌酐明显升高,且肾功能受损程度与窒息程度成正比(P<0.05).早产窒息儿较足月窒息儿更易发生肾功能损伤(P<0.05).结论 围产期窒息患儿早期进行心肌酶学和肾功能检测,对相关异常早诊断、早治疗,对疾病预后的判断及治疗具有重要意义.
