针刺结合强迫疗法治疗脑卒中后上肢运动功能障碍的临床研究

摘要

目的：观察针刺结合强迫疗法治疗脑卒中后上肢运动功能障碍的疗效。方法：将脑卒中后上肢运动功能障碍的60例患者随机分为对照组30例和治疗组30例，两组患者均给予神经科基本治疗和康复科基础治疗（包括肌力训练、降张训练、神经促通技术治疗）。治疗组在上述治疗基础上采用针刺拮抗肌配合强迫疗法治疗，对照组在上述治疗基础上单纯加用强迫疗法治疗，治疗后对两组患者肢体痉挛状态及神经功能缺损程度进行评定。结果：在肢体肌张力改善方面，治疗组与对照组比较差异有统计学意义（ P＜0．05）。在STEF和神经功能缺损程度改善方面，两组治疗后分别与治疗前比较，差异均有统计学意义（P＜0．05）；治疗后治疗组与对照组比较，差异有统计学意义（P＜0．05）。结论：针刺拮抗肌结合强迫疗法的治疗方法较单纯运用强迫疗法治疗卒中后上肢运动功能障碍疗效明显有效。%Objective:To observe the effect of acupuncture combined with forced therapy on upper limb motor dysfunction after stroke .Methods:60 patients with upper limb motor dysfunction after stroke were randomly di-vided into a control group with 30 patients and a treatment group with 30 patients, and the two groups were giv-en neurological basic treatment and rehabilitation therapy ( including strength training , reduced muscle tension training , neural facilitation technique therapy ) .In the treatment group , it was based on the use of acupuncture treatment with forced antagonist therapy , and the control group was on the basis of the above treatment plus forced therapy alone .The two groups after eight weeks of treatments among patients with limb spasticity and neu-rological deficits were evaluated .Results:The improvement in limb muscle tension in the treatment group was compared with that in the control group ,and the difference was statistically significant (P<0.05); STEF and the improvement of neurological deficit after treatment were compared with pre -treatment,and the differences were statistically significant ( P<0 .05 );the efficacy of treatment group was compared with that of control group , and the difference was statistically significant ( P<0.05 ) .Conclusion:Acupuncturing at antagonrstic muscles combined with forced therapy for upper limb motor dysfunction after stroke is more obvious and effective than forced therapy alone .
