颈部牵引联合手法复位治疗寰枢关节半脱位临床观察

摘要

Objective To observe the clinical curative effect of cervical traction combined with manipulative reduction in the treatment of atlantoaxial subluxation.Methods The clinical data of 162 patients with atlantoaxial subluxation in our department from 2011 to 2015,who were treated by cervical traction combined with manipulative reduction were retrospectively analyzed.The clinical symptoms and X-ray changes before and after treatment were observed.Results All patients received 5 days of cervical traction combined with manipulative reduction treatment,32 cases were cured,effective in 122 cases,ineffective in 8 cases,the total effective rate (cure + effective) was 95.1％.After treatment,X-ray atlantoodontoid before the gap value (ADI) and atlanto backlash (VBLADS) had statistically significant difference compared with before treatment (P＜0.01).Conclusion The cervical traction combined with manipulative reduction in the treatment of atlantoaxial subluxation is effective,and it is worthy of promotion.%目的 观察颈部牵引联合手法复位治疗寰枢关节半脱位的临床疗效.方法 回顾性分析我科2011-2015年门诊、病房采用颈部牵引联合手法复位治疗的寰枢关节半脱位患者162例,观察其治疗前后临床症状及X线表现变化情况.结果 所有患者经过5d颈部牵引联合手法复位治疗,治愈32例,有效122例,无效8例,总有效率(治愈+有效)95.1％.治疗后X线片寰齿前间隙值(ADI)和寰齿侧间隙差值(VBLADS)和治疗前相比有统计学意义(P均＜0.01).结论 颈部牵引联合手法复位治疗寰枢关节半脱位的方法疗效确切,值得推广.
