血小板计数和D-二聚体水平联合检测在肺癌中的临床意义*

摘要

Objective: To study the clinical significance of separate or combined detection of platelet count and D-dimer level in peripheral blood in lung cancer. Methods: Clinical data of 107 previously untreated lung cancer patients and 94 healthy people were collected. The two groups were compared in clinical data, platelet count and D-dimer level. And we cal-culated sensitivity, specificity, Youden index and area under curve ( AUC) of separate or combined detection of platelet count and D-dimer level to determine the clinical significance of platelet count and D-dimer level in lung cancer. Results:Platelet count and D-dimer level were significantly higher in the observation group than those in the control group ( P <0. 05). In the observation group, platelet count and D-dimer level of patients with Ⅲ-Ⅳ stage tumors were significantly higher than those of patients with I-Ⅱ stage tumors ( P < 0. 05). Sensitivity and AUC of combined detection of platelet count and D-dimer level were higher than those of separate de-tection of platelet count and D-dimer level. Conclusion: Separate or combined detection of platelet count and D-dimer level in lung cancer has certain clinical significance in the diagnosis of lung cancer, and the clinical significance of combined de-tection is higher.%目的:探讨外周血中血小板(platelet, PLT)计数和D-二聚体(D-dimer, D-D)水平单项或联合检测在肺癌中的临床意义.方法:收集107例初治肺癌患者和94例健康体检者的临床资料,对两组研究对象的临床资料及PLT计数、D-D水平进行比较,通过计算 PLT 计数、D-D 水平单项或联合检测的灵敏度、特异度、约登指数、曲线下面积(AUC),发现PLT计数及D-D水平在肺癌中的临床意义.结果:PLT计数、D-D水平在观察组明显高于对照组,差异有统计学意义(P<0. 05).在观察组中,Ⅲ~Ⅳ期肺癌患者的PLT计数、D-D水平明显高于I~Ⅱ期患者,差异有统计学意义(P<0. 05). PLT计数、D-D联合检测的灵敏度、曲线下面积高于单项检测.结论:PLT计数、D-D水平单项或联合检测在肺癌中均有一定的临床价值,联合检测的临床价值相对较高.
