经腹腹腔镜与后腹腔镜治疗腹侧肾囊肿疗效比较

Objective To investigate the best choice of laparoscopic renal cyst surgery approach in the treatment of ventral renal cysts. Method 60 cases of patients with ventral renal cyst receiving laparoscopic renal cyst unroofing were included in this study. The 60 cases were divided into 2 groups randomly. One group received transperitoneal laparoscopic renal cyst unroofing,and the other group re-ceived retroperitoneal approaches. The revealing time of cysts,the whole time of operation,bleeding volume,the incidence of complication, and post-operation hospitalization duration were compared. Results Compared with the retroperitoneal approach group,the transperitoneal group had less revealing time of cysts. On the other hand,the transperitoneal group had more average operative time,the average bleeding volume,the incidence of complication and the average hospitalization duration after operation. Conclusion Lapamscopic renal cyst unroofing has become a routine surgery method of renal cyst,even though in the treatment of ventral renal cysts.%目的：探讨经腹腹腔镜与后腹腔镜治疗腹侧单纯性肾囊肿的治疗效果。方法：选取行腹侧单纯性肾囊肿去顶减压术的患者共60例，随机分为两组，分别采用经腹腹腔镜与后腹腔镜手术治疗。对两种手术路径的囊肿暴露时间、手术时间、术中出血量、手术并发症、术后恢复情况等进行比较。结果：经腹腹腔镜组囊肿暴露时间显著低于后腹腔镜组，但是手术时间、术中出血量、肠道功能恢复时间、手术并发症发生率及术后住院时间均高于后腹腔镜组（ P<0.05）。结论：后腹腔镜肾囊肿去顶术已成为肾囊肿手术治疗的标准术式，即使囊肿位于腹侧，后腹腔镜也有其优势，对于术前考虑囊肿暴露困难的患者亦可考虑经腹路径。
