MiRNA-196a在牙周炎及牙龈炎中的表达及作用研究

Objective To explore and analyze the expression and effect of MiRNA-196a in periodontitis and gingivitis.Methods 45 patients with gingivitis and periodontitis treated at our department from August,2016 to August,2018 were selected as clinical research objects,and were divided into a gingivitis group and a periodontitis group.The periodontitis group were divided into three groups (15 cases for each group) according to the severity of periodontitis:mild periodontitis,moderate periodontitis,and severe periodontitis.Forty healthy volunteers who came to the hospital for physical examination in the same period were selected as a control group.The gingival sulcus was extracted from all the research objects to detect the miR-196a expression,and their plaque index (PLI),gingival index (GI),probing depth (PD),attachment loss (AL),and other clinical indicators were detected.The correlation of miR-196a expression with PLI,GI and PD,AL,etc.in all the groups were analyzed.Results The expression level of miR-196a in gingival sulcus was significantly higher in the periodontitis group than in the gingivitis group and the healthy control group [(3.22±0.67),(1.45±0.53),and (0.67±0.21);P ＜ 0.05],and was significantly higher in the severe periodontitis group than in the moderate periodontitis group and the mild periodontitis group [(4.35±0.68),(3.41±0.74),and (2.34±0.58);P ＜ 0.05].The PLI,GI,PD,and AL were significantly higher in the periodontitis group and the gingivitis group than in the healthy control group (all P ＜ 0.05),and were (3.73±0.72),(3.55±0.74),(6.32±0.72) mm,and (5.21±0.57) mm in the in the severe periodontitis group,which were higher than those in the moderate periodontitis group and the mild periodontitis group (all P ＜ 0.05).The content of mir-196a in gingival sulcus fluid of patients with gingivitis or periodontitis significantly and positively correlated with PLI,GI,PD,AL,and other clinical indicators (P ＜ 0.05).Conclusions mir-196a is highly expressed in gingival crevicular fluid in patients with gingivitis or periodontitis.The degree of periodontitis increases with mir-196a expression,suggesting that the expression level of mir-196a in gingival crevicular fluid can be used as an effective evaluation indicator for the diagnosis and treatment of periodontitis.%目的 探究分析MiRNA-196a在牙周炎及牙龈炎中的表达及作用研究.方法 选取2016年8月至2018年8月期间本院口腔科收治的牙龈炎及牙周炎患者90例作为临床研究对象,各45例.分别记为牙龈炎组、牙周炎组,并按牙周炎严重程度将试验B组患者分为轻度牙周炎、中度牙周炎及重度牙周炎三组,各15例.选取同期来院体检的40例健康志愿者作为对照组.分别提取研究对象龈沟液,检测其龈沟液中miR-196a的表达,同时检测研究对象菌斑指数(PLI)、牙龈指数(GI)、探诊深度(PD)、附着丧失(AL)等临床指标,分析不同研究对象龈沟液miR-196a表达水平与PLI、GI、PD、AL等临床指标间的相关关系.结果 牙周炎组患者龈沟液miR-196a表达水平(3.22±0.67),明显高于牙龈炎组(1.45±0.53)及健康对照组(0.67±0.21),差异有统计学意义(P＜0.05);且重度牙周炎组患者龈沟液中miR-196a表达水平(4.35±0.68),明显高于中度牙周炎组(3.41±0.74)及轻度牙周炎组患者(2.34±0.58),差异有统计学意义(P＜0.05);牙龈炎组及牙周炎各组患者PLI、GI、PD、AL等临床指标水平均明显高于健康对照组,差异均有统计学意义(均P＜ 0.05);且重度牙周炎组患者PLI、GI、PD、AL等临床指标水平(3.73±0.72)、(3.55±0.74)、(6.32±0.72) mm、(5.21±0.57) mm,明显高于轻度、中度牙周炎组,差异均有统计学意义(均P＜ 0.05);牙龈炎及牙周炎患者龈沟液miR-196a含量与PLI、GI、PD、AL等临床指标水平呈明显正相关,差异有统计学意义(P＜0.05).结论 牙龈炎及牙周炎患者龈沟液中miR-196a呈明显高表达,且随着miR-196a表达水平的升高,其牙周炎程度更为严重,提示龈沟液miR-196a表达水平可作为临床牙周炎诊治的有效评估指标.
