CXCR4在卵巢上皮性癌组织和转移淋巴结中的表达及临床意义

摘要

Objective To analyze the expression of CXC chemokine receptor 4 (CXCR4) in epithelial ovarian cancer and associated lymph node metastases,and investigate the relationship between their clinicopathological features variables and prognosis.Methods Using a tissue microarray of 85 epithelial ovarian cancer tissues (including 18 cases of metastatic lymph node tissues) coupled to a comprehensive database of clinicopathological variables as well as benign epithelial ovarian tumors and normal ovarian tissues,the expression of CXCR4 was assessed by immunohistochemistry.Its impact in terms of survival and clinicopathological variables was also determined.Results Higher CXCR4 expression was found in metastatic lymph node and epithelial ovarian cancer tissues were compared to benign epithelial ovarian tumors and normal ovarian tissues.Expression of CXCR4 was correlated with clinical stage,lymphatic metastasis and distant metastasis.Moreover,patients with higher expression of CXCR4 were associated with shorter survival time.Conclusion Expression of CXCR4 in epithelial ovarian cancer is correlated with the patients' clinicopathological variables and prognosis.CXCR4 might play a role in epithelial ovarian cancer progression,could be a symbol index of invasion,transferring and prognosis.%目的 探讨趋化因子受体4(CXC chemokine receptor 4,CXCR4)在原发性卵巢上皮性癌组织和转移淋巴结中的表达与患者的临床病理特征及预后的关系.方法 收集东莞市人民医院妇产科在2002年1月至2008年1月间手术切除的85例卵巢上皮性癌组织(其中18例同时收集到转移淋巴结组织)、25例卵巢良性上皮性肿瘤组织、25例正常卵巢组织存档蜡块用于组织芯片制作.采用免疫组织化学技术检测上述组织中的CXCR4的表达,分析其与患者的临床病理特征及预后的相关性.结果 CXCR4在正常卵巢组织、卵巢良性上皮性肿瘤组织、卵巢上皮性癌组织和转移淋巴结组织中表达的阳性率分别为8％、40％、67％和83％,呈递增趋势.其中CXCR4在卵巢上皮性癌组织和转移淋巴结组织中的表达与正常卵巢组织相比差异有统计学意义(P＜0.01).CXCR4的表达水平与卵巢上皮性癌患者的肿瘤临床分期、淋巴结转移及远处转移等重要的临床病理参数及患者术后生存时间相关.结论 CXCR4在卵巢上皮性癌组织及转移淋巴结组织中均呈高表达,且与肿瘤临床分期、淋巴结转移及远处转移等临床病理参数相关,提示CXCR4可能参与卵巢上皮性癌的发生与进展,有望成为预测卵巢上皮性癌侵袭、转移和预后的指标.
