新生儿TSH水平与孕妇单纯TPOAb水平升高的关系分析

摘要

目的 通过对新生儿促甲状腺激素(TSH)和孕妇甲状腺过氧化物酶抗体(TPOAb)的筛查,了解孕妇妊娠早期单纯TPOAb水平升高是否影响新生儿的甲状腺功能.方法 选择30例甲状腺功能正常及妊娠早期TPOAb正常的孕妇所生新生儿作为对照组,选择30例甲状腺功能正常及TPOAb升高(TPOAb>34 IU/L)孕妇所生新生儿为观察组.分析两组新生儿TSH是否存在差异以及孕妇TPOAb水平与新生儿TSH的关系.结果 30例观察组新生儿的TSH水平为(2.91±0.10)mIU/L,显著高于30例对照组新生儿的TSH水平(1.92±0.18)mIU/L,差异有统计学意义(t=-42.29,P<0.001);相关性分析显示,孕妇TPOAb水平与新生儿TSH水平呈正相关(r=0.46).结论 妊娠早期孕妇TPOAb水平升高可能会影响新生儿甲状腺功能.%Objective By screening the neonatal thyroid stimulating hormone (TSH) and the thyroid peroxidase antibody (TPOAb) in pregnant women, to investigate whether the simple-elevated level of TPOAb in pregnant women during the first trimester of pregnancy affects the thyroid function of the newborn or not. Methods 30 cases of neonates whose mothers had normal thyroid function and normal TPOAb level were selected as control group, 30 cases of neonates whose mothers had normal thyroid function and elevated level of TPOAb ( > 34 IU/L) were selected as observation group. And then analyzed the difference of neonatal TSH level between the two groups, and the relationship between TPOAb level in pregnant women and neonatal TSH level. Results The neonatal TSH level in the observation group was (2.91±0.10) mIU/L, significantly higher than that in the control group [(1.92±0.18) mIU/L], with statistically significant difference between the two groups (t=-42.29, P<0.001). The correlation analysis showed that the TPOAb level in pregnant women was positively correlated with the neonatal TSH level (r=0.46). Conclusion The elevated level of TPOAb in pregnant women during the first trimester of pregnancy may affect the neonatal thyroid function.
