集约化管理在住院患者出院带药管理中的应用价值

摘要

目的 探讨集约化管理在住院患者出院带药管理中的应用价值.方法 本院于2015年4月起对住院患者出院带药管理实施集约化管理,选取2015年4月—2016年3月(流程改进后)需出院用药的住院患者31500例,另选取2014年4月—2015年3月(流程改进前)需出院用药的住院患者20000例.统计流程改进前后患者接受出院用药医嘱情况、用药医嘱不合格情况及患者满意度并进行比较.结果 流程改进后患者用药指导及用药医嘱接受率为100.00%,高于流程改进前的85.86%,差异有统计学意义(P<0.05).流程改进后患者出院药方及用药医嘱不合格率为0%(0/31500),低于流程改进前的8.51%(1702/20000),差异有统计学意义(P<0.05).流程改进后患者对该流程满意率100.00%(31500/31500),患者综合满意度为95.26%(30007/31500),均高于流程改进前的94.62%(18924/20000)和92.99%(18598/20000),差异有统计学意义(P<0.05).结论 住院患者出院带药集约化管理有效整合了出院带药办理环节,提高了工作效率,显著降低了出院带药处方和医嘱的不合格率,使专业指导用药和出院用药医嘱得到有效落实,加强了用药安全性,患者满意度明显提高.%Objective To investigate the effect of intensive management in discharge medication management of in-patients. Methods The intensive management was carried out in discharge medication management of in-patients of our hospital since April, 2015. The compliance rate of medication guidance, the unqualified rate of medical prescription, the satisfaction of patients were observed before and after implementation of intensive management (between April, 2014 and March, 2015, 20000 patients; between April, 2015 and March, 2016, 31500 patients). Results The compliance rate of medication guidance after implementation of intensive management was higher than that before implementation of intensive management (100.00% vs.85.86%), with statistically significant difference (P<0.05). The unqualified rate of medical prescription after implementation of intensive management was lower than that before implementation of intensive management (0% vs.8.51%), with statistically significant difference (P<0.05). The satisfaction of patients on the process and overall satisfaction after implementation of intensive management were higher than those before implementation of intensive management [100.00% (31500/31500) vs. 94.62% (18924/20000), 95.26% (30007/31500) vs. 92.99% (18598/20000)], with statistically significant differences (P<0.05). Conclusion The intensive management in discharge medication management of in-patients can effectively optimize discharge link, improve the work efficiency and patients' satisfaction degree.
