缺血性脑卒中患者脂蛋白相关磷脂酶A2、同型半胱氨酸、高敏C反应蛋白联合检测的意义

摘要

目的 分析缺血性脑卒中(ICS)患者血浆同型半胱氨酸(Hcy)、超敏C反应蛋白(hs-CRP)和脂蛋白相关磷脂酶A2(Lp-PLA2)的水平及联合检测的意义.方法 选取ICS患者140例纳入ICS组,健康体检者100例纳入对照组,检测血浆Hcy、hs-CRP、Lp-PLA2水平,统计三者阳性率,分析血浆Hcy、hs-CRP、Lp-PLA2水平及联合检测对ICS诊断的意义.结果 ICS组血浆Hcy、hs-CRP、Lp-PLA2水平及阳性率均高于对照组,差异有统计学意义(P<0.05).联合检测血浆Hcy、hs-CRP、Lp-PLA2水平诊断ICS的受试者工作特征曲线下面积为0.949,灵敏度为0.850,特异度为0.807,均高于3项指标单独检测.结论 ICS患者血浆Hcy、hs-CRP、Lp-PLA2水平均升高,三者联合检测可提高对ICS早期诊断价值.%Objective To analyze the significance of levels and combined detection of plasma homocysteine(Hcy),high-sensitivity C reactive protein(hs-CRP) and lipoprotein-associated phospholipase A2(Lp-PLA2) in patients with ischemic cerebral stroke(ICS).Methods A total of 140 patients with ICS were selected into ICS group,and 100 healthy individuals were recruited in the control group.The plasma levels of Hcy,hs-CRP,Lp-PLA2 were examined,then the positive rates of them were statistically analyzed.Analyzed the value of the levels and joint detection of Hcy,hs-CRP and Lp-PLA2.Results The plasma levels and the positive rates of Hcy,hs-CRP and Lp-PLA2 in ICS group were statistically higher than those in the control group(P<0.05).The area under the receiver-operating characteristic curve of combined detection of Hcy,hs-CRP and Lp-PLA2 in diagnosis of ICS was 0.949,the sensitivity was 0.850,and the specificity was 0.807,which were higher than Hcy,hs-CRP and Lp-PLA2 alone.Conclusion The plasma levels of Hcy,hs-CRP and Lp-PLA2 increase in ICS patients,and the combined detection of the three indicators might improve the early diagnostic value of ICS.
