参芎葡萄糖注射液对急性缺血性卒中患者阿司匹林治疗期间血小板反应性的影响

摘要

目的 探讨参芎葡萄糖注射液对急性缺血性卒中患者阿司匹林治疗期间血小板反应性的影响.方法 前瞻性纳入2014年1月至2016年12月期间12家医院收治的急性缺血性卒中患者,随机分为阿司匹林组和参芎葡萄糖注射液+阿司匹林组.检测两组治疗前后4种血小板诱聚剂(花生四烯酸、二磷酸腺苷、胶原和血小板活化因子)诱导的血小板最大聚集率变化.结果 阿司匹林组(n=132)人口统计学资料和基线临床特征与参芎葡萄糖注射液+阿司匹林组(n=131)差异均无统计学意义.基线时,参芎葡萄糖注射液+阿司匹林组花生四烯酸和血小板活化因子诱导的血小板最大聚集率显著高于阿司匹林组(P均<0.05).治疗后第6天,参芎葡萄糖注射液+阿司匹林组4种诱聚剂诱导的血小板最大聚集率均显著低于阿司匹林组(P均<0.001).结论 参芎葡萄糖注射液对急性缺血性卒中患者阿司匹林治疗期间血小板反应性具有显著的抑制作用.%Objective To investigate the effect of Shenxiong glucose injection on platelet reactivity during aspirin treatment in patients with acute ischemic stroke.Methods A total of 263 patients with acute ischemic stroke admitted to 12 hospitals from January 2014 to December 2016 were enrolled prospectively.They were randomly divided into aspirin group and aspirin + Shenxiong glucose injection group.The changes of platelet maximum aggregation rate induced by 4 platelet aggregating agents (arachidonic acid,adenosine diphosphate,collagen and platelet activating factor) were detected before and after the treatment.Results There were no significant differences in the demographic data and baseline clinical characteristics between the aspirin group (n =132) and the Shenxiong glucose injection + aspirin group (n =131).At baseline,the maximum aggregation rate of platelet induced by arachidonic acid and platelet activating factor in Shenxiong glucose injection + aspirin group was significantly higher than that in the aspirin group (all P <0.05).On the 6th day after treatment,the maximum aggregation rate of platelets induced by the 4 aggregating agents in the Shenxiong glucose injection + aspirin group was significantly lower than that in the aspirin group (all P < 0.001).Conclusion Shenxiong glucose injection had a significant inhibitory effect on platelet reactivity during aspkin treatment in patients with acute ischemic stroke.
