沙丁胺醇联合布地奈德对重症肺炎合并呼吸窘迫综合征患者心功能及血气分析的影响

             

摘要

目的 探究在重症肺炎合并呼吸窘迫综合征中采用沙丁胺醇联合布地奈德治疗对患者心功能及血气的影响.方法将2017年1月—2018年1月间在吴川市妇幼保健院接受治疗的60例重症肺炎合并呼吸窘迫综合征患儿作为研究对象.按患儿入院顺序分为对照组 (n=30) 与治疗组 (n=30) .对照组单独采用沙丁胺醇治疗, 治疗组采用沙丁胺醇联合布地奈德治疗.观察两组治疗前后血气指标和心功能指标及临床效果.结果 两组治疗前各项血气指标与心功能指标比较差异无统计学意义 (P>0.05);治疗后, 治疗组二氧化碳分压 (PaCO2) 水平低于对照组, 氧分压 (PaO2) 、血氧饱和度 (SpO2) 水平高于对照组 (P<0.05);治疗组IVST、LVEDD、LVESD水平低于对照组, LVEF水平高于对照组 (P<0.05) .治疗组病死率、肺损伤发生率低于对照组, 且成功撤机率、机械通气时间高 (短) 于对照组 (P<0.05) .结论 沙丁胺醇联合布地奈德可显著提高重症肺炎合并呼吸窘迫综合征的治疗效果, 恢复患者心肺功能, 值得临床推广与使用.%Objective To investigate the effects of salbutamol combined with budesonide on cardiac function and blood gas in patients with severe pneumonia complicated with respiratory distress syndrome.Methods 60 cases of severe pneumonia complicated withrespiratory distress syndrome treated in the hospital from January, 2017 to January, 2018 were regarded was research objectives.According to their admission order, they were divided into two groups:control group (n = 30) and treatment group (n = 30). The con-trol group were treated with salbutamol alone, and the treatment group were treated salbutamol combined with budesonide. The bloodgas index was observed, including arterial blood carbon dioxide partial pressure (PaC) before and after treatment in two groupscardiacfunction and clinical efficiency.Results There was no significant difference in blood gas index and cardiac function index between thetwo groups before treatment (P> 0.05). After treatment, the level of PaCO2 in the treatment group was lower than that in the controlgroup (P< 0.05), and the level of PaO2 and SpO2 in the treatment group was higher than that in the control group (P< 0.05). The lev-el of LVEF, LVEDD and LVESD was lower than that of control group (P< 0.05). The mortality and incidence of lung injury in thetreatment group were lower than those in the control group, and the probability of successful withdrawal was higher than that in thecontrol group (P< 0.05), and the mechanical ventilation time was shorter than that in the control group (P< 0.05).Conclusion Sal-butamol combined with budesonide can significantly improve the therapeutic effect of severe pneumonia complicated with respiratorydistress syndrome and restore the cardiopulmonary function of the patients, so it is worth popularizing and using in clinic.
