肾窦内肾盂及肾后下段联合切开治疗肾巨鹿角形结石的疗效观察

摘要

目的:探讨肾窦内肾盂及肾后下段联合切开治疗肾巨鹿角形结石的临床疗效及安全性.方法:对47例肾巨鹿角形结石患者采取肾窦内肾盂及肾后下段联合切开的方法进行治疗.结果:47例患者平均手术时间为(153±7.6)min,术中出血量为(268±109) mL,术中不需阻断肾血流,结石最大为(7.2×5.6×3.4)cm,其中1例患者最多取出结石74枚.术后1年随访,经B超或KUB+IVU复查,所有患者肾功能恢复良好,3例患者发现肾内残余结石,直径≤0.5cm.结论:肾窦内肾盂及肾后下段联合切开治疗肾巨鹿角形结石安全有效,具有操作简单、出血量少、取石干净、肾损伤小等优点,适合临床推广应用.%Objective:To study the clinical curative effect and security of intrarenal sinus lower segment joint incision in treating renal giant staghorn calculi.Method:47 cases of renal giant staghorn calculi patients were with the incision of intrarencal sinus and postrenal low segement.Result:47 patients average operation time was (153 + 7.6) min,intraoperative amount of bleeding for (268 + 109) mL,intraoperative does not need to block the renal blood flow,the biggest of calculus was (7.2×5.6×3.4) cm,one of the patients took stones 74 pieces.Postoperative 1 year of follow-up,by examination of B-ultrasonography or KUB + IVU review,all patients renal function recovered well,3 patients were found in the kidney stone residual,diameter acuities were 0.5 cm.Conclusion:Renal sinus in renal pelvis and after lower segment joint incision in treating renal giant staghorn shape stone is safe and effective,with simple operation,less amount of bleeding,stone clean,the advantages of small renal injury,suitable for clinical application.
