前十字韧带重建术股骨侧与胫骨侧骨隧道区域骨密度的比较

     

摘要

目的:对比分析前十字韧带重建术股骨侧与胫骨侧骨隧道区域的骨密度。方法:选取2012年1月至2013年4月间于我院进行体检的健康者20例,同时选取在此期间由于前十字韧带损伤而接受前十字韧带重建术的患者11例,采用双能X线吸收骨密度测试仪与薄层CT扫描测定所有健康者的股骨侧与胫骨侧的DEXA骨密度及骨隧道区域CT灰度值,同时于前十字韧带重建术中对患者的股骨侧与胫骨侧隧道内松质骨体积骨密度进行测定。结果:健康者前股骨侧的DEXA骨密度与骨隧道区域CT灰度值均明显高于胫骨侧{(1.159±0.029)g/cm3,(210.8±12.1)Hu vs (0.808±0.041) g/cm3,(105.3±8.2)Hu, t=31.257,32.279 P=0.000,0.000};11例患者中,股骨侧隧道内松质骨体积骨密度要明显高于胫骨侧{(2.793±0.872)g/cm3 vs (1.913±0.605)g/cm3 t=2.750 P=0.012}。结论:前十字韧带重建术股骨侧骨隧道区域的骨密度要高于胫骨侧,临床上在使用螺钉时可以此为依据进行选择。%Objective:To compare the bone mineral density in the regions of femoral and tibial tunnel during anterior cruciate ligament reconstruction .Methods:From Jan.2012 to Apr.2013, 20 volunteers were enrolled in the study ,and the DEXAl was used to measure the bone mineral density of femoral and tibial tun-nel regions of the knees .All the knees of the volunteers were also scanned by spiral CT , and three-dimen-sional reconstruction technique was utilized to determine the circular sections that pass through the longitudi -nal axis of the femoral and tibial tunnels separately .The CT gray scale values of the sections were measured . 11 patients who had been diagnosed as ACL rupture underwent the operation of ACL reconstruction , and cy-lindrical cancellous bone peg was removed from the femoral and tibial tunnel respectively during the opera -tion.Volumetric bone mineral density of the bone pegs was measured .Result: The bone mineral density measured by DEXA and CT gray scale of femoral region of the healthy people was statistically higher than that of the tibial region {(1.159±0.029)g/cm3, (210.8±12.1)Hu vs (0.808±0.041)g/cm3, (105.3±8.2) Hu, t=31.257, 32.279 P=0.000, 0.000}.Among the 11 patients, the volumetric bone mineral density of the bone peg from femoral tunnel was statistically higher than that from the tibial tunnel .Conclusion: For ACL reconstruction , bone mineral density of femoral tunnel region is higher than that of the tibial tunnel , which could provide reference for the choice of optimal screw diameter for interference fixation .
