高通量血液透析对血清 β2-微球蛋白和甲状旁腺激素的影响

         

摘要

目的:探讨高通量血液透析对血清β2-微球蛋白和甲状旁腺激素的影响. 方法:60例血液透析患者随机分为两组,对照组予常规低通量透析,观察组予高通量透析,比较两组患者治疗前后血清β2-微球蛋白(β2-MG)、甲状旁腺素(PTH)变化. 结果:两组患者治疗开始时透析前血清β2-MG、PTH比较差异无统计学意义(P>0.05),对照组治疗开始首次透析后血清β2-MG较治疗前无明显变化( P>0.05) ,观察组透析后血清β2-MG、PTH均较透析前明显降低,且均低于对照组水平,差异有统计学意义(P<0.05);4周后,对照组血清β2-MG、PTH较治疗前仍无明显变化(P>0.05),但是观察组血清β2-MG、PTH均较治疗前明显降低,且均低于对照组水平,差异有统计学意义( P<0.05). 结论:高通量血液透析可有效降低血清β2-MG、P TH水平,可能对减少透析相关性淀粉样变、肾性骨病及心血管疾病等并发症的发生有利.%Objective:To investigate th effect of high flux hemodialysis on serum β2-microglobulin( β2-MG) and parathyroid hormone( PTH) .Method:60 cases of hemodialysis patients were randomly divided into two groups, the control group was treated with conventional low-flux dialysis, and the observation group was treated with high flux dialysis,before and after treatment,the level of serum β2-MG, PTH was compared between the two groups.Result:There was no statistically difference in β2-MG, PTH between two groups at the beginning (P>0.05), and no statistically difference in control group of serum β2-MG compared with before the frist hemodialysis ( P>0.05) ;but the serum β2-MG, PTH in observation group after dialysis was significantly lower than before dialysis, and the levels were lower than the control group, the difference was statistically significant ( P <0.05);after 4 weeks, there were no significant change of serum β2-MG, PTH in the control group(P>0.05), but the serum β2-MG, PTH in observation group was significantly lower than before treatment, and the levels were lower than the control group, the difference was statistically signif-icant (P <0.05).Conclusion:High-flux hemodialysis can effectively reduce serum β2-MG, PTH levels in hemodialysis patients,and it can reduce dialysis-related amyloidosis, renal osteodystrophy and beneficial car-diovascular disease and other complications.
