术后早期肠内营养对老年食管癌患者免疫功能和预后的影响

摘要

Objective:To study the effect of early enteral nutrition on the immunal function and pros-pect of elderLY patients with esophageal cancer.Method:100 patients from January 2011 to December 2013 were divided into 2 groups, 50 cases in study group(EEN), and 50 cases in control group(TPN).Compared the clinical measurement, gastrointestinal function, complication, nutrition condition and immunal function between two groups.Result: The difference of blood loss, operation time and liquid drainage between 2 groups has no statistical significance( P>0.05) .The difference of length of stay and cost has statistical signifi-cance( P<0.05) .The difference of exhaust time and defection time has statistical significance( P<0.05) , the rate of gastrointestinal reaction and complication between 2 groups has no statistical difference( P>0.05) .In study group, the weight loss is(-0.24±0.87)kg, while in control group is(0.79±1.23)kg , the difference has no statistical significance( P>0.05) .After 1 week, the difference of ALB and PA between 2 groups has statistical siginificance(P<0.05).The difference of immunoglubin, CD3+, CD4+and CD4+/CD8+between 2 groups has statistical siginificance( P<0.05) .Conclusion:The early enteral nutrition can improve the nutri-tion condition and immunal function of patients after operation, as well as shorten the lengthy of stay.%目的：研究术后早期肠内营养对老年食管癌患者免疫功能和预后的影响。方法：选取2011年1月至2013年12月期间收治的100例食管癌患者作为研究对象，将100例患者随机分为两组，其中观察组50例，采取早期肠内营养（EEN）；对照组50例，采取胃肠外营养（TPN），比较两组患者的临床指标、胃肠功能和术后并发症、营养状态和免疫功能。结果：两组患者术中出血量、手术时间、术后48h的液体引流量无统计学差异（ P＞0．05），患者术后的住院时间和住院花费具有统计学差异（ P＜0．05）。两组患者的术后首次排气时间和排便时间具有统计学差异（ P＜0．05），患者的胃肠道反应和并发症发生率无统计学差异（P＞0．05）。观察组患者的体重损失（－0．24±0．87）kg，对照组患者体重损失（0．79±1．23）kg，无统计学差异（P＞0．05）。两组患者术后1周的ALB和PA水平具有统计学差异（P＜0．05）。术后1周，两组患者的血清免疫球蛋白和CD3＋、CD4＋和CD4＋／CD8＋具有统计学差异（ P＜0．05）。结论：早期的肠内营养可以促进患者营养状态和免疫功能的恢复，缩短住院时间。
