低蛋白血症对食管癌术后并发症的影响

         

摘要

Objective:To study the the influence of hypoalbuminemia on postoperative complications of esophageal cancer through statistical methods .Method:From Jan.2008 to Jun.2012, 100 cases of patients with esophageal cancer whole medical records data were collected for statistical analysis .Based on esophageal cancer patients with postoperative at 3, 5, 7 days and 1 day before leaving hospital four times low serum al-bumin levels, divided into severe low serum albumin leels (serum alnumin <25g/L), medium low serum al-bumin levels group (serum alnumin 25~30g/L), mild low serum albumin levels group (serum alnumin 30~34g/L) group and normal albumin (serum alnumin ≥ 35g/L).For each concurrent infection of incision , postoperative anastomotic fistula , the incidence of cardiovascular system and respiratory system complications for data statistics.Using SPSS17.0 software for statistical processing , counted data by chi-square test, the difference was statistically significant ( P<0.05) .Analysed serum albumin level and postoperative infection of incision, postoperative anastomotic fistula , cardiovascular system and respiratory complications .Result:Postoperative incision infection of severe low serum albumin levels group , the anastomotic fistula , the inci-dence of cardiovascular system and respiratory system complications was significantly higher than the other three groups, and with serum albumin levels rise , postoperative incision infection , anastomotic fistula , the incidence of cardiovascular system and respiratory system complications decreased obviously .Conclusion:The hypoalbuminemia patients of esophageal cancer are more likely to appear all sorts of serious complica -tions , and it should be given full attention and effective intervention in order to reduce the incidence of com-plications and promote the patient recovey soon .%目的:通过统计学方法探讨低蛋白血症对食管癌术后患者并发症发生的影响。方法:收集2008年1月至2012年6月行手术治疗且所需资料完整的食管癌患者100例进行回顾性分析。依据患者术后第3天、第5天、第7天以及出院前最后1次血浆白蛋白水平的最低值,分为重度低白蛋白组(ALB<25 g/L ),中度低白蛋白组( ALB 25~30g/L),轻度低白蛋白组( ALB 30~34g/L )和正常白蛋白组( ALB≥35g/L)。对各组术后呼吸系统、心血管系统、切口感染、吻合口瘘等并发症的发生率进行数据统计。采用SPSS17.0软件进行统计学处理,计数资料采用χ2检验,以P<0.05为差异有统计学意义。分析低蛋白血症对食管癌术后患者并发症的影响。结果:术后低白蛋白组呼吸系统并发症、切口感染、吻合口瘘的发生率明显高于正常白蛋白组,且随着血浆白蛋白水平的上升,以上三种并发症的发生率明显下降,但对心血管系统及其他并发症影响无统计学意义( P >0.05)。结论:食管癌术后并发低白蛋血症患者更易出现各种严重并发症,应充分重视并给予有效的干预,以降低并发症发生率,促进患者早日康复。
