棒皮砂莲方外敷联合棒皮苦胆四妙汤口服治疗痛风性关节炎急性期临床研究

摘要

目的 观察棒皮砂莲方外敷联合棒皮苦胆四妙汤口服治疗痛风性关节炎急性期的临床疗效.方法 将100例痛风性关节炎急性期患者随机分为2组.对照组50例予秋水仙碱片联合依托考昔片口服治疗;治疗组50例在对照组治疗基础上加棒皮砂莲方外敷联合棒皮苦胆四妙汤口服治疗.2组均连续治疗1周.比较2组治疗前后血尿酸 (UA) 、红细胞沉降率 (ESR) 及C反应蛋白 (CRP) 水平变化和中医证候评分变化, 统计比较2组临床疗效及不良反应.结果 治疗后2组UA、ESR及CRP水平均较本组治疗前明显降低 (P＜0.05), 且治疗组低于对照组 (P＜0.05) .治疗后2组各中医证候评分均较本组治疗前明显降低 (P＜0.05), 且治疗组低于对照组 (P＜0.05) .治疗组总有效率 (92.00％) 高于对照组 (74.00％, P＜0.05), 不良反应发生率 (8.00％) 低于对照组 (12.00％, P＜0.05) .结论 棒皮砂莲方外敷联合棒皮苦胆四妙汤口服治疗痛风性关节炎急性期能明显改善患者关节疼痛、肿胀症状, 降低UA、ESR及CRP水平, 疗效确切, 用药安全.%Objective To observe the clinical effects of external application of Bangpi Shalian prescription and oral administration of Bangpi Kudan Simiao decoction on acute gouty arthritis. Methods 100 patients with acute gouty arthritis were randomly divided into two groups. 50 cases in control group were treated by colchicine tablets combined with etocoxib tablets. 50 cases in treatment group were treated by Bangpi Shalian prescription and oral administration of Bangpi Kudan Simiao decoction on the basis of the control group. Both groups treated for one week. The changes of serum uric acid (UA), erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) levels and TCM syndrome scores were compared before and after treatment between two groups. The clinical efficacy and adverse reactions of the two groups were statistically compared. Results The levels of UA, ESR and CRP in the two groups were significantly lower than those in the group before treatment (P＜0.05), and the treatment group was lower than the control group (P＜0.05). The scores of TCM syndromes in the two groups were significantly lower than those in the group before treatment (P＜0.05), and the treatment group was lower than the control group (P＜0.05). The total effective rate of the treatment group (92.00％) was higher than that of the control group (74.00％, P＜0.05), and the incidence of adverse reactions (8.00％) was lower than that of the control group (12.00％, P＜0.05). Conclusion External application of Bangpi Shalian prescription and oral administration of Bangpi Kudan Simiao decoction can significantly improve the symptoms of joint pain and swelling, and reduce the levels of UA, ESR and CRP in patients with gouty arthritis in acute stage. The curative effect is definite and the medication is safe.
