Objective To explore the clinical effect of compound danshen dropping pills (CDDP) in patients with acute myocardial infarction (AMI) and to explore the mechanism of CDDP in the inflammatory response mediated by NF-κBp65. Methods 124 patients with AMI were randomly divided into the routine (n = 58) and CDDP group (n = 66). After 30 d, the LVEF, NF-κBp65, hs-CRP, TNF-α, IL-1, IL-6, IL-10 of patients with AMI were compared with the healthy people. Results Before the treatment, expression of NF-κBp65, hs-CRP, TNF-α, IL-1, IL-6 and IL-10 in routine group and CDDP group were higher than those of the control group (P< 0.05). The level of LVEF was less than that in the control group (P< 0.05). After the treatment, the expression levels of NF-κBp65, hs-CRP, TNF-α, IL-1, IL-6 and IL-10 of the patients with AMI were decreased significantly (P< 0.05), while the LVEF level was increased significantly (P< 0.05). And the results in CDDP group were better than the routine group (P< 0.05). Conclusion CDDP can inhibit the inflammatory response and improve the cardiac function by regulating the expression level of NF-κBp65 and hs-CRP, and inhibiting the release of inflammatory factors and oxygen free radicals. CDDP may be used to treat AMI and the inflammatory responses.%目的 探讨复方丹参滴丸用于治疗急性心肌梗死及其引起的炎症反应的临床疗效,并探索其抗炎机制.方法 将我院收治的124例急性心肌梗死患者随机分为两组,常规治疗组58例,常规治疗联合复方丹参滴丸组(CDDP组)66例,连续治疗30 d后,对比分析对照组(健康体检者)和两组急性心肌梗死患者左室射血分数(LVEF)的变化,以及血清中核因子NF-κBp65、高敏C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)、IL-1、IL-6和IL-10的表达情况.结果 常规治疗组和CDDP组治疗前NF-κBp65、hs-CRP、TNF-α、IL-1、IL-6和IL-10的表达水平均高于对照组(P<0.05),左室射血分数明显低于对照组(P<0.05).治疗后,两组患者的NF-κBp65、hs-CRP、TNF-α、IL-1、IL-6和IL-10的表达水平均较治疗前明显降低(P<0.05)、LVEF明显升高(P<0.05),且CDDP组各指标的变化程度均大于常规治疗组(P<0.05).结论 复方丹参滴丸能够通过调节NF-κBp65转录因子和hs-CRP的表达,抑制炎症因子及氧自由基的释放,起到抑制炎症反应、改善心功能的作用,可用于治疗急性心肌梗死.
