脑瘫患儿摄食和胃肠功能与粗大运动功能分级相关性

         

摘要

目的 探讨脑性瘫痪(脑瘫)患儿摄食技能和胃肠功能特点及其与粗大运动功能分级的关系.方法 对2010年6月至2011年10月在西安市儿童医院行康复治疗的80例脑瘫患儿及行健康体检的80例正常儿童进行摄食技能、胃肠功能调查及粗大运动功能评估,并对数据进行统计学分析.结果 脑瘫组与对照组的摄食技能障碍、胃肠功能紊乱发生率比较差异均有统计学意义(χ2值分别为8.01、5.38,均P<0.05),其中摄食技能分比较差异有统计学意义(t=7.23,P<0.05).两组流涎、厌食、便秘发生率比较差异均有统计学意义(χ2值分别为18.57、7.81、4.10,均P<0.05);两组平均用餐时间比较差异有统计学意义(t=4.51,P<0.05).脑瘫粗大运动功能分级轻度-中度组与重度组的流涎、厌食、便秘发生率比较差异均有统计学意义(χ2值分别为10.20、12.00、11.10,均P<0.05),摄食技能分比较差异有统计学意义(t=4.40,P<0.05);脑瘫粗大运动功能分级轻度-中度组平均用餐时间1.8±0.39(h/d)与重度组2.5±0.52(h/d)比较差异有统计学意义(t=9.10,P<0.05).结论 脑瘫患儿临床上有显著的摄食技能障碍和胃肠功能紊乱,其影响患儿的体格发育及康复治疗的进展.%Objective To investigate the feeding and gastrointestinal function problems in children with cerebral palsy ( CP ) and their relationship with gross motor function classification ( GMFC ). Methods The feeding ability and gastrointestinal function of 80 children with CP treated in Xi' an Children' s Hospital from June 2010 to October 2011 and those of 80 normal children receiving health examination were investigated and their gross motor function were evaluated. All of the data was statistically analyzed. Results The differences in the incidence of feeding dysfunction and gastrointestinal disorders between CP group and control group were statistically significant (x2 value was 8. 01 and 5. 38, respectively, both P < 0. 05 ), and the difference in feeding ability was significant ( t = 7. 23 , P < 0. 05 ). The incidence of salivation, anorexia and constipation in two groups were significantly different (x2 value was 18. 57, 7. 81 and 4. 10, respectively, all P < 0. 05 ), so was the average dining time ( t = 4. 51 , P <0. 05). There were statistical differences in incidence of salivation, anorexia and constipation between mild-moderate group and severe group according to GMFC (x2 value was 10.20, 12.00 and 11. 10, respectively, all P <0. 05 ) and in feeding ability score ( t = 4. 40, P < 0. 05 ). The average dining time was 1.8-0. 39( h/d )in mild-moderate group and 2.5-0. 52( h/d )in severe group, and the difference was significant-(t=9. 10,P<0. 05). Conclusion CP children have prominent manifestations of feeding dysfunction and gastrointestinal disorders, which affect the physical development and rehabilitation of them.
