ET技术评价小剂量短期雌激素替代治疗绝经妇女颈动脉弹性

     

摘要

目的 采用高频彩色多普勒超声诊断(echo-tracking,ET)技术评价小剂量短期雌激素替代治疗对绝经后妇女颈动脉弹性功能的影响.方法 50例绝经妇女经小剂量雌激素替代治疗3个月;28例拒绝雌激素替代治疗妇女为非治疗组;25例正常行经妇女为对照组.非治疗组与治疗前后对比观察血清雌二醇(E2)、一氧化氮(NO)、促卵泡生成素(FSH)、黄体生成素(LH)、泌乳素(PRL)及血脂变化,采用ET技术检测对照组、非治疗组及替代治疗前后妇女颈动脉血管内径的变化幅度、弹性僵硬指数(β)、压力应变弹性系数(Ep)、动脉顺应性(AC)、脉搏波放大指数(AI)及单点脉搏波传递速度(PWVβ).结果 小剂量短期雌激素替代治疗可明显升高血E2和NO水平,降低FSH、LH水平,可降低血管僵硬度,增加血管弹性及顺应性指数,改善绝经后妇女颈动脉弹性功能,且与血脂的改善无显著相关.结论 应用ET技术可准确评价绝经后妇女小剂量短期雌激素替代治疗前后颈动脉的弹性功能.%Objective Using E-tracking technology to evaluate carotid artery elasticity on postmenopausal women low-dose short-term estrogen replacement therapy with. Methods 50 cases of postmenopausal women with low-dose estrogen replacement therapy for 3 months, 28 patients refused estrogen replacement and 25 normal women were included. Serum estradiol (E2), nitric oxide (NO) , follicle-stimulating EPO (FSH) , luteinizing hormone (LH) , prolac-tin (PRL) and serum lipid profiles were detected before and after the estrogen treat ment. The carotid artery diameter changes, carotid intima-media thickness (IMT) , elastic stiffness index (β) , pressure strain elastic modulus (Ep) , arterial compliance (AC) , pulse wave amplification index ( AI) and the single point of pulse wave velocity (PWVβ) were obtained by ET. Results Short-term low-dose estrogen replacement therapy could increase the level of E2 and NO, reduce FSH, LH levelsto reduce vascular stiffness parameters in postmenopausal women with no improvement in blood lipids. Conclusions E-tracking technology can be an accurate assessment of carotid artery elasticity on postmenopausal women with low-dose short-term estrogen replacement therapy.
