超声引导下经皮微波消融治疗肝转移癌近期疗效及影响因素

摘要

Objective To investigate the appli attim value of ultrasound-guided percutaneous microwave ablation for metastasic liver carcinoma (MLC) and short-term effects factors.Methods The clinicopathological date of 111 patients were retrospectively analyzed,the factors of short-term effects were explored.Results 111 patients with 324 metastasic liver carcinomas were treated and 158 ultrasound-guided percutaneous microwave ablation sessions were performed.245 of 324 carcinomas(75.6 ％) achieved complete ablation.The Chi-square test results showed that the diameter of tumors,the distance from hepatic surface,gastrointestinal trace and secondary and nore secondary bile duct hepatic vessels were significantly associated with the complete ablation rate(P＜ 0.05).The multiple variables logistic regression analysis showed that only the diameter of tumors was associated with the complete ablation rate(P＜0.05).Conclusions Ultrasound-guided percutaneous microwave ablation is a feasible,safe and effective method for metastasicliver carcinoma.The clinical value is significant.%目的 探讨超声引导下经皮微波消融治疗肝转移癌应用价值及近期疗效影响因素.方法 回顾性分析111例158次超声引导下经皮微波消融治疗肝转移癌的临床资料,记录分析近期疗效,探讨影响疗效的因素.结果 对111例324个病灶进行了 158次微波消融治疗.完全消融245个,完全消融率75.6％.单因素分析结果显示:经皮微波消融治疗转移性肝癌近期疗效与肿瘤的直径,肿瘤与肝脏表面、胃肠道和二级及以上肝内胆管、血管距离有关(均P＜0.05).Logistic回归分析结果显示:经皮微波消融治疗转移性肝癌近期疗效仅与肿瘤的直径有关(P均＜0.05).结论 超声引导下微波消融治疗肝转移癌是一种可行、安全、有效的方法,而且近期疗效确切,具有重要的临床价值.
