不同刺激声对颈性前庭诱发肌源性电位的影响

         

摘要

Objective To investigate the effects of different acoustic stimuli on cervical vestibular evoked myogenic potential (cVEMP) in healthy subjects. Methods A total of 30 healthy adult volunteers were recruited as subjects and 500 Hz tone burst, 750 Hz tone burst and click were employed as stimuli for cVEMP examinations in bilateral ears of each subject. The response rate, P1 latency, N2 latency, P1-N2 latency interval, amplitude and inter-aural asymmetry of induced cVEMP were compared among groups. Results The response rate of cVEMP to 500 Hz tone burst was 100%, which was significantly higher than that to 750 Hz tone burst (70.0%) or that to click (73.3%), P<0.05. P1-N2 amplitude of cVEMP induced by 500 Hz tone burst was the largest (82.28±38.58μV), which was than that by 750 Hz tone burst (46.02±20.18μV, P<0.05) or that by click (77.99±44.59μV, P<0.05). There were no significant differences in P1 latency, N2 latency, P1-N2 latency interval, amplitude and inter-aural asymmetry of cVEMP induced by different acoustic stimuli (P>0.05). Conclusion Saccule responses to acoustic stimuli, with frequency characteristics. cVEMP are affected by different acoustic stimulus and 500Hz tone burst was the best stimulus for inducing cVEMP, with a good response rate, the biggest amplitude and most easily recognizable waveform.%目的 观察不同种类的刺激声对正常人气导声刺激诱发的颈性前庭诱发肌源性电位(cVEMP)的影响.方法 选择30例正常人为研究对象,分别采用500Hz短纯音、750Hz短纯音和短声作为诱发刺激声进行双耳(60耳)cVEMP检测,观察和比较cVEMP波形的引出率、P1潜伏期、N2潜伏期、P1-N2波间期、振幅及耳间对称性情况.结果 500Hz和750Hz的短纯音与短声刺激诱发的cVEMP引出率分别为100%、70%和73%,500Hz短纯音与其他两组间的引出率差异均有统计学意义(P<0.05);500Hz短纯音诱发的cVEMP振幅最大,500Hz与750Hz的短纯音、750Hz短纯音与短声所诱发的cVEMP振幅差异均有统计学意义(P<0.05).P1和N2的潜伏期、P1-N2波间期及不对称比在500Hz和750Hz的短纯音与短声刺激之间差异均无统计学意义(P>0.05).结论 球囊具备一定的频率特性,不同刺激声可影响cVEMP波形,500Hz短纯音诱发的cVEMP引出率高,振幅较大,波形易辨认,应针对不同刺激声诱发的cVEMP建立不同的标准值.
