局部联合应用糖皮质激素治疗乳突根治术后术腔肉芽的疗效观察

摘要

目的 观察局部联合应用糖皮质激素治疗乳突根治术后术腔肉芽的疗效.方法 回顾性分析2013年5月~2017年5月在我科行乳突根治术且术后复查发现术腔有新生肉芽的患者27例,男16例,女11例,年龄3~75岁,耳内镜下先用刮匙将患者术腔肉眼可见的肉芽尽可能刮除干净,用地塞米松纱条局部压迫,24小时后取出,然后嘱患者局部喷洒鼻用丙酸氟替卡松,其中4例患者因肉芽范围太大连续行2次清理和地塞米松纱条压迫直至肉芽大部消退再行局部喷洒丙酸氟替卡松,分别于2周、1月、3月后再次复诊.结果 所有患者经局部联合治疗(地塞米松纱条压迫+喷洒鼻用丙酸氟替卡松)后肉芽消退,术腔上皮化.结论 局部联合应用糖皮质激素治疗乳突根治术后术腔肉芽操作简单,安全可靠,疗效确切.%Objective To observe the curative effect of the local combined application of glucocorticoid in the treatment of granulation tissue after radical mastoidectomy. Methods The clinical data of 27 patients (16 males, 11 females) who were found granulation in the surgical cavities after radical mastoidectomy aged 3-75 from May 2013 to May 2017 were analyzed retrospectively. Firstly, we cleared the granulation as clean as possible under aurisendoscopic, pressed the wound with glucocorticoid sponges immediatedly which were moved after 24 hours, then sprayed fluticasone propionate, and asked them to visit after two weeks、one month and three months respectively, but 4 cases of patients cleared the granulation twice and pressed with glucocorticoid sponges until the vast majority of granulation subsided. Results The granulation tissue of all patients subsided after the treatment of the local combined application of glucocorticoid, the surgical cavity brought to stage of epithelization. Conclusions The local combined application of glucocorticoid in the treatment of granulation tissue after radical mastoidectomy is simple, safe and reliable.
