种植支持全牙弓和分段式修复无牙颌患者对成功率、骨吸收及美观评价的影响

摘要

Objective: To study the effects of full-arch dental implant restoration and sectional repair for success rate, bone resorption and aesthetic evaluation of edentulous mandible. Methods: To retrospective analysis datum of 64 patients with edentulous mandible, divide into 2 groups according to different repair ways. 31 patients that used sectional repair were defined as the control group, 33 patients that used full-arch dental implant restoration were defined as the observation group. Compared the success rate, bone resorption and aesthetic evaluation of the two groups. Results: There were no differences in the success rate of implant and repair, the bone resorption of 3, 4 months and 1 year after implant between the two groups (P>0.05). The aesthetic evaluation scores of observation group (4.72±0.26) were observably higher than the control group (3.56±0.25)(P<0.01) . No differences were observed in degree of comfort and masticatory function between the two groups (P>0.05). Conclusions:The full-arch dental implant restoration and sectional repair for edentulous mandible both had a high success rate, and with a certain degree of bone resorption either. But the maintenance convenience and aesthetic evaluation of full-arch dental implant restoration were better than the control group, and had a better clinical promotion value.%目的：研究无牙颌者应用种植支持全牙弓与分段式修复对成功率、骨吸收及美观评价的影响。方法：资料选取本院2011年5月~2015年5月收治的无牙颌64例患者予以回顾性地分析，按照临床所用不同修复方法分为2组，将应用种植支持分段式修复31例患者作为对照组，将应用种植支持全牙弓修复33例患者作为观察组，对两组成功率、骨吸收及美观评价情况进行对比。结果：两组种植成功率、修复成功率、种植后3、4个月和修复结束后1年骨吸收状况对比均未显示高度差异（P＞0.05）；观察美观评价评分（4.72±0.26）分比对照组（3.56±0.25）分高（P＜0.01）；两组舒适度、咀嚼功能评分对比均无明显差异（P＞0.05）。结论：无牙颌者无论应用种植支持全牙弓或者分段式修复的种植与修复成功率均较高，且均出现一定骨吸收，但种植支持全牙弓修复在维护方便性和美观评价等方面均比对照组优，具有临床推广价值。
