玻璃体切除联合气体充填术后高眼压的处理

摘要

Objective To investigate the treatment of postoperative ocular hypertension after pars plana vitrectomy with perfluoropropane gas tamponade.Methods From January 2009 to June 2010,in our department 43 cases(43eyes) of patients with intraocular hypertension after vitrectomy with perfluoropropane gas tamponade were treated with local and systemic intraocular pressure(IOP) lowering therapy,and six cases were combined with paracentesis.Results IOP of 10 cases dropped to 10 ～21 mmHg within one week without any medication.IOP of 27 cases were controlled to 10 ～ 21 mmHg with medication.Six cases achieved 10 ～ 21 mmHg IOP with medication and paracentesis of anterior chamber or vitreous cavity.Followed up for 2 ～4 months,all cases had 10 ～ 21 mmHg IOP without any sever complication.Conclusion It is necessary and helpful to control ocular hypertension after pars plana vitrectomy with perfluoropropane gas tamponade to achieve safe IOP and protect visual function.%目的 探讨玻璃体切除联合全氟丙烷气体(C3F8)充填术后高眼压的处理方法.方法 对43例玻璃体切除联合C3F8填充术后高眼压者采取观察和应用局部、全身降眼压治疗,并对6例采取穿刺术处理.结果 10例未用降眼压药物,眼压1周内恢复正常(10 ～21 mm Hg),27例应用药物治疗眼压恢复正常,6例经前房和或玻璃体腔穿刺,眼压恢复正常.术后随访2～4个月眼压均在正常范围,未出现严重并发症.结论 玻璃体切除联合C3F充填术后高眼压经过及时恰当的降眼压治疗,眼压均能恢复正常,从而有效地保护了视功能.
