钼钯X线、超声及二者组合BI-RADS分类在农村妇女乳腺筛查中的对比分析

         

摘要

目的 对比分析钼钯X线(MAM)、超声(US)及二者组合在农村妇女乳腺筛查中BI-RADS分类的异同,探讨其分类评价价值.方法 选取在本院接受乳腺筛查的本地区农村妇女,年龄35~60岁,制定入选标准,954名入选.对所有受检者先行US检查,再行MAM检查;参照美国放射学会提出的乳腺影像报告和数据系统2003年第4版分类标准进行US、MAM及US+ MAM分类,对所得结果行统计学分析,比较3种分类方法的差异.结果 统计学分析显示3种方法所得Ⅰ、Ⅱ类结果的受检者年龄及人数差异有统计学意义(P<0,05),Ⅲ、ⅣA类受检者的年龄差异无统计学意义(P>0,05);Ⅲ~V类受检者数差异无统计学意义(P>0.05).结论 US+ MAM组合法BI-RADS分类可增加农村妇女乳腺Ⅱ类结果检出率;对于Ⅲ~ⅣB类结果,MAM较US检出率高.%Objective To analyze the difference of mammography (MAM), ultrasonography (US) and their combination used for breast screening in countryside women, and to explore their classification value. Methods Totally 954 local countryside women aged 35 to 60 years underwent breast screening including MAM and US. The results were classified according to the breast imaging reporting and data system (BI-RADS) proposed by American college of radiology in 2003. BI-RADS classification of MAM, US and MAM+US were compared statistically. Results Statistical significant differences of aging and numbers of subjects were found among MAM,US and MAM+US for class I and FJ (P<0. 05). No significant difference of aging existed for class ffl and IV A among the three methods (P>0. 05) , nor of the numbers of subjects for class Ⅲ-Ⅳ (P>0. 05). Conclusion BI-RADS classification of MAM+US can increase detection rate of class II for breast screening in countryside women. Compared with US, BI-RADS classification of MAM can increase the detection rate for class Ⅲ- Ⅳ B.
