彩色多普勒血流显像评价兔肾缺血再灌注后肾动脉血流动力学变化及其与肾组织髓过氧化物酶活性的关系

         

摘要

目的 运用彩色多普勒血流显像(CDFI)评价兔肾缺血再灌注(I/R)后肾动脉血流动力学变化,探讨其与肾组织髓过氧化物酶(MPO)活性变化间的相关性.方法 将56只大白兔随机分为I/R组、假手术组、正常对照组,I/R组及假手术组又各分为3个亚组.应用CDFI分别监测各组肾主动脉(MRA)、段动脉(SRA)、叶间动脉(IRA)的血流动力学变化,并测定肾组织MPO活性行相关分析.结果 与正常对照组比较,I/R 2 h亚组无明显血流动力学改变,I/R 8 h亚组SRA及IRA出现阻力指数(RI)增大(P均<0.05),I/R 24 h亚组MRA、SRA和IRA均出现收缩期峰值流速(Vmax)、搏动指数(PI)、RI增大(P均<0.05);假手术组血流动力学均无明显改变.I/R组MPO活性升高,且24 h内有加重趋势(P<0.05);假手术组MPO活性无明显改变.各组MRA、SRA和IRA的Vmax、PI、RI与肾组织MPO活性变化呈正相关(P均<0.05).结论 兔肾I/R 24 h内肾动脉血流动力学发生改变,且组织末端血管最先出现改变;兔肾I/R 24 h内MRA、SRA和IRA的Vmax、PI、RI与肾组织MPO活性变化呈正相关.CDFI是评价兔肾I/R 24 h内肾动脉血流动力学变化的有效方法.%Objective To observe the renal artery hemodynamics changes in rabbits with renal ischemia/reperfusion (I/R) using color Doppler flow imaging (CDFI), and to explore the correlation with myeloperoxidase (MPO) activity. Methods Fifty-six rabbits were divided into normal, sham-operated and I/R groups randomly. The I/R group and the shamopemted group were divided into 3 subgroups, respectively. The hemodynamic changes of master renal artery (MRA),segment renal artery (SRA) and interlobar arterial renal (IRA) were monitored, and the correlation with MPO activity was analyzed. Results Compared with normal group, sham-operated and I/R 2 h subgroup had no significant changes in hemodynamic. In I/R 8 h subgroup, resistance index (RI) of SRA and IRA increased (both P<0. 05), while in I/R 24 h subgroup, maximum velocity of systolic phase (Vmax), pulsatility index (PI), RI of MRA, SRA and IRA increased (all P<0. 05). Compared with normal group, MPO activity had no significant changes in sham-operated group and elevated in I/R group, especially within 24 h (P<0.05). Vmax, PI, RI of MRA, SRA and IRA had positive correlation with MPO activity (all P<0. 05). Conclusion The renal artery hemodynamic changes after I/R within 24 h in rabbits, and occurs firstly in the blood vessels at the end of the tissue. Vmax, PI, RI of MRA, SRA and IRA have positive correlation with MPO activity. CDFI is an effective method to valuate renal artery hemodynamic changes within I/R 24 h in rabbits.
