颅面结构立体可视化和颅面整形手术仿真模拟的研究

摘要

Objective In order to achieve the computerization ,visualization, standardization and quantitatization of diagnosis, surgical planning and surgical result assessing in craniomaxillofacial plastic surgery . Methods Based on multimedia microcomputer we developed a integrated sof tware package of computer-assisted three-dimensional visualization, quantitative diagnosis and simulation system for craniomaxillofacial surgery (3DCMFCAS) by using Visual C++ language and volume data visualizing algorithm . The 3D CMFCAS system includes several functional modules : CT image inputting module , registering and format converting module , three-dimensional measurement and analysis module , quantitative simulation module , as well as archive of patients . Through many times experiments on different methods of 3-D reconstruction , the surface rendering method that suitable for craniomaxillofacial plastic surgery was chosen as the 3-D model reconstruction method , and a new volume data manipulat ing method spacial subregion volume data manipulation was put forward as the operation simulation method that could be operated speedily in microcomputer environment.Results By means of the 3DCMFCAS system and related methods , the three-dimensional model of craniofacial anatomic structures and malfor mations were reconstructed for qualitative observation and quantitative analysis .The craniomaxillofacial osteotomy procedures were successfully simulated on the 3DCMFCAS system platform . Conclusions The application of 3 DCMFCAS system indicates that the computerized system provide update instrumenta lity and practical , effective methods for fundamental research , diagnosis and surgical therapy of craniomaxillofacial malformations .%目的为研究颅面畸形的诊断分析、手术设计、疗效预测及颅颌面生长发育监测的数字化、定量化和可视化。方法应用Visual C ++编程语言和计算机图形图像三维可视化算法原理,研制出颅面CT图像三维重建、立体结构三维测量和颅面整形手术模拟的系统软件。建立了以CT数据为信息源基于多媒体微型计算机的颅面畸形三维仿真模拟系统(3DCMFCAS)达到诊断分析、手术设计的可视化。选用了适合于颅面整形外科特点的面向多边形的表面绘制显示方法作为系统建模方法;提出了适合于在微型计算机环境中进行手术模拟操作的空间子区域目标结构体数据操作方法。结果在3DCMFCAS系统上构建出颅面骨、软组织立体结构模型,建立了颅面图像及电子解剖数据库,实现了经典颅面截骨整复手术仿真模拟。结论 3DCMF CAS系统方法的建立及应用为颅面外科临床诊断和整形手术设计提供了实用有效的技术手段。
