二维斑点追踪技术评价系统性红斑狼疮亚临床左心室收缩功能不全

     

摘要

Objective To explore the value of two-dimensional speckle tracking imaging (STI) in evaluation on subclinical left ventricular systolic dysfunction in patients with systemic lupus erythematosus (SLE). Methods Totally 30 patients with SLE (SLE group) and 20 healthy persons (control group) were studied. Longitudinal mean strain (LS), mean strain rate (LSR) and time to peak mean strain (TPLS) in apical four chamber view and circumferential mean strain (CS) , mean strain rate (CSR) and time to peak mean strain (TPCS) in short axis view of the papillary muscle level were measured by STI. Results Comparing to control group, LS, CSR and CS were significantly lower in SLE group (all P<0. 05). There was no statistical difference of LSR, LSTP nor CSTP between the two groups (all P>0. 05). Conclusion STI can detect the subclinical left ventricular systolic dysfunction and provide some evidence for the pathophysiology of myocardial involvement in patients with SLE.%目的 探讨二维斑点追踪成像技术(STI)评价系统性红斑狼疮(SLE)患者亚临床左心室收缩功能不全的临床价值.方法 收集30例SLE患者(SLE组)及20名健康志愿者(对照组),应用STI检测心尖四腔心左心室长轴平均应变(LS)、平均应变率(LSR)及平均应变达峰时间(TPLS)和乳头肌水平短轴切面平均圆周应变(CS)、平均应变率(CSR)及平均应变达峰时间(TPCS).结果 SLE组LS、CSR及CS均显著低于对照组(P均<0.05),两组间LSR、LSTP及CSTP的差异均无统计学意义(P均>0.05).结论 STI技术能够检测出SLE患者亚临床型左心室收缩功能异常,并为探讨其心肌受累的病理生理机制提供线索.
