动脉内注射巴曲酶治疗犬急性脑梗死的实验研究

             

摘要

目的 探讨动脉内注射巴曲酶治疗急性脑梗死的有效性及安全性.方法 成年实验犬16只,随机分为实验组与对照组.用介入技术建立犬急性脑梗死模型,两组于建模6h后用微导管经颈内动脉分别注入巴曲酶5 BU及生理盐水10 mL,用药结束后30 min、60 min及120 min分别行脑血管造影复查,检查血管再通情况.同时两组分别在溶栓前30 min及溶栓后120 min抽取静脉血,测定凝血酶原时间(PT)、活化的部分凝血活酶时间(APTT)及纤维蛋白原(FIB)水平.结果 实验组与对照组治疗有效的血管数分别为10支和1支,总有效率分别为83.33%和0.09%,两组比较有统计学意义(P<0.01).溶栓后实验组PT、APTT、FIB与对照组比较无统计学意义;实验组溶栓前后PT、APTT、FIB比较无统计学意义.24 h内无严重并发症.结论 动脉内注射巴曲酶治疗急性脑梗死是安全、有效的.%Objective To study the efficacy and safety of intra - arterial batroxobin thrombolytic therapy for acute cerebral infarction in dog. Methods Sixteen adult dogs were randomly divided into control group and experimental group. The models with acute cerebral infarction were established with interventional embolization technique. Following postembolization 6 hours,5 BU batroxobin was injected by the micro - catheter through the internal carotid artery and 10 mL saline was injected in control group. At 30 min,60 min and 120 min after treatment, cerebral angiography was performed to check the situation of recanalization. The levels of prothrom-bin time(PT) .activated partial thromboplastin time( APTT)?thrombin time(TT) ,fibrinogen(FIB) were assayed at 30 min before thrombolysis, and at 120 min after thrombolysis. Results There were 10 recanaled vessels (83. 33%) in experimental group,and there was 1 (0. 09%) in control group. There was difference in two groups statistically(P<0. 01). The levels of PT, APTT and FIB in two groups showed no statistically differences. The levels of PT, APTT and FIB in experimental group were no significant differences before and after thrombolysis. There were no serious complications in 24 hours. Conclusion Intra - arterial injection of batroxobin for acute cerebral infarction is safe and effective.
