非O血型通过升高高敏C反应蛋白及血脂水平增加冠心病及心肌梗死的发病风险

Objective To investigate the association of non O blood group with coronary heart disease (CHD)and myocardial infarc-tion (MI)and possible mechanisms. Methods A total of 304 consecutive CHD patients,149 MI patients and 201 controls were en-rol ed. The ABO blood groups,high sensitive C reactive protein (hs CRP),serum lipid profiles and other biological variables were measured. Results Compared with control group,CHD group [OR=2.09,95% CI (1.32,3.29),P<0.01]and MI group[OR=2.09, 95% CI (1.32,3.29),P<0.01]had more proportions of non O blood group. The hs CRP,total cholesterol (TC),low density lipoprotein cholesterol (LDL C)and non high density lipoprotein cholesterol(NHDL C)were significantly higher while high density lipoprotein cholesterol (HDL C)were significantly lower in CHD and MI group than in control group(P<0.01). The hs CRP was significantly higher in MI group than in CHD group(P<0.01). Hs CRP,TC,LDL C,and NHDL C were significantly higher in non O blood group than in O blood goup(P<0.05). Multivariate logistic regression analysis showed that non O group,hs CRP,male gender,age were the independent risk factors for CHD. Conclusion Non O group can increase the risk of CHD and MI might through elevated the levels of hs CRP and serum cholesterol.%目的：探讨非 O血型对冠心病及心肌梗死发病的影响及其可能的机制。方法连续入选住院的冠心病病人304例,其中心肌梗死(MI)149例；选择同期住院的非冠心病病人201例作为对照组。测定所有入选病例的 ABO血型、血清高敏 C反应蛋白(hs CRP)及血脂[总胆固醇(TC)、高密度脂蛋白(HDL C)、低密度脂蛋白(LDL C)、非高密度脂蛋白(NHDL C)]等各项生化指标。同时按血型将入选对象分为非 O型与 O型。结果与对照组比较,冠心病组[OR=2.08,95%CI (1.43,3.02),P<0.01]及 MI组[OR=2.09,95%CI (1.32,3.29),P<0.01]非 O型病人比例明显升高；与对照组比较,冠心病组及 MI组 hs CRP、TC、LDL C、NHDL C显著升高,HDL C明显下降(P<0.01)；与冠心病组比较,MI组 hs CRP明显升高(P<0.01)；与 O型病人比较,非 O型病人 hs CRP、TC、LDL C、NHDL C明显升高(P<0.05)。多元Logistic回归分析显示,非O血型、hs CRP、男性、年龄是冠心病发病的独立危险因素。结论非 O血型、hs CRP是冠心病发病的独立危险因素,非 O血型通过升高 hs CRP及血脂水平增加冠心病及 MI的发病风险。
