Objective To explore the effects of levocarnitine (LC) and atorvastatin calcium (AC) on cardiac function,serum C-reactionprotein (CRP),interieukin (IL)-6,and tumor necrosis factor (TNF)-α in patients with chronic heart failure (CHF).Methods One hundreds and forty-four patients with CHF in our hospital from June 2015 to June 2017 were randomly divided into two groups:control group (n =72) treated with AC,and treatment group (n =72) treated with AC and LC for 4 weeks.The clinical effect,the changes of cardiac function and serum CRP,IL-6,TNF-α levels were observed before and after treatment,as well as adverse reaction were observed.Results After 4 weeks of treatment,the total clinical effective rate was significantly higher in treatment group than that in control group (91.7％ vs 69.0％,P ＜0.01).Left vertricular end diastolic diameter(LVEDD)/left vertricular end systolic diameter (LVESD),and serum CRP,IL-6,TNF-α levels decreased while left ventricular ejection fraction (LVEF) increased in two groups after treatment (P ＜0.01).The cardiac function was better in treatment group than that in control group (P ＜ 0.01).There was no difference in the adverse reaction rate between treatment group and control group (4.2％ vs 5.6％,P ＞0.05).Conclusion LC and AC can relieve the symptoms and signs and improve the cardiac function effectively in patients with CHF.It may be related to the improvement of serum CRP,IL-6 and TNF-α levels in patients.%目的 探讨左卡尼汀(LC)联合阿托伐他汀钙(AC)治疗慢性心力衰竭(CHF)的临床效果及对心功能和血清C反应蛋白(CRP)、白细胞介素(IL)-6、肿瘤坏死因子-α(TNF-α)水平的影响.方法 选取我院2015年6月-2017年6月收治的144例CHF病人,采取随机数字表法分为两组.对照组(72例)予以AC治疗,观察组(72例)在此基础上加用LC治疗.比较两组临床疗效,治疗前后心功能指标及血清CRP、IL-6,TNF-α水平变化,不良反应发生情况.结果 治疗4周后,观察组总有效率为91.7％,较对照组显著升高(69.0％,P＜0.01).与治疗前比较,两组治疗4周后心功能指标左心室舒张/收缩末期内径(LVEDD/LVESD)值及血清CRP,IL-6,TNF-α水平均显著降低(P＜0.01),左心室射血分数(LVEF)值均显著升高(P＜0.01);且观察组以上指标改善更显著(P＜0.01).与对照组(5.6％)相比,观察组不良反应率为4.2％,差异无统计学意义(P＞0.05).结论 左卡尼汀联合阿托伐他汀钙治疗CHF更能有效缓解病人症状体征,改善心功能,疗效更佳,可能与其显著改善血清中CRP,IL-6,TNF-α水平有关,且安全可靠.
