目的 探讨自然受孕、辅助生殖技术及服用促排卵药3种方式受孕双胎妊娠的不同妊娠结局.方法 选取2013年7月至2014年6月期间在济宁医学院附属医院分娩孕妇324例,其中辅助生育技术受孕双胎孕妇(辅助生殖组)106例,自然受孕双胎孕妇(自然受孕组)81例,服用促排卵药物受孕(促排卵药组)137例.对3组双胎妊娠孕妇的围生期结局进行回顾性分析.结果 辅助生殖组产妇妊娠期糖尿病、胎膜早破、难免早产发生率明显高于其他两组(P＜0.05);妊娠期高血压疾病、前置胎盘、胎盘早剥、S/D异常、羊水异常、一胎胎死宫内、剖宫产、胎儿畸形、低体重儿出生率、新生儿窒息发生率3组比较,差异无统计学意义(P＞0.05).结论 与自然双胎妊娠相比,应用辅助生殖技术受孕双胎孕妇妊娠期糖尿病、胎膜早破、难免早产等妊娠期并发症明显增多,剖宫产率和新生儿结局无明显影响.%Objective To study different pregnancy outcomes in twin pregnancy of natural fertility and assisted reproductive technology and take ovulation stimulating drug.Methods A total of 324 cases with twin pregnancy hospitalized in Affiliated hospital of Jining medical college from July 2013 to June 2014 were selected,in which 106 cases assisted by reproductive technology were selected as assisted reproductive group,81 cases with natural conception were selected as natural fertility group,137 cases with stimulated ovulation drugs were selected as stimulate ovulation medicine group.Different outcomes in the perinatal period of 3 groups were analyzed retrospectively.Results The incidence rate of gestational diabetes,premature rupture of membranes,unavoidable premature birth in assisted reproductive group were obviously higher than the other two groups (P ＜ 0.05);Gestational hypertension,placenta previa,placental abruption,abnormal S/D,an abnormal amniotic fluid,a dead fetus intrauterine,cesarean section rate,low birth weight,newborn asphyxia of three groups had no significant difference (P ＞ 0.05).Conclusion Compared with natural twin pregnancy,the incidence rates of gestational diabetes,premature rupture of membranes,unavoidable premature delivery and other pregnant complications after application of assisted reproduction increase obviously,cesarean section rate and neonatal outcome has no obvious influence.
