脂蛋白脂酶Ser447Ter基因变异和吸烟对缺血性卒中的影响

摘要

Objective To investigate the influence of interaction of lipoprotein lipase ( LPL ) gene Ser447Ter variation and smoking on the risk of ischemic stroke. Methods The case-control design method was used to study 96 cases of ischemia stroke and 117 cases of non-cardiovascular and cerebrovascular diseases. The distribution of LPL gene Ser447Ter variation and smoking in stoke patients and the influence of their interaction on risk of stroke were observed by applying polymerase chain reaction-restriction fragment length polymorphism ( PCR-RFLP ). Results The proportion of the patients carrying LPL Ser447Ter was 10. 4% , which was significantly lower than that in control people group ( 21.4% ,P<0.05 ). The odds ratio ( OR ) for stroke was 0.428 ( 95% CI:0. 194 ～0. 994;P =0. 035 ) in the patients carrying Ser447Ter,and was 3. 08 ( 95% CI:2. 105 ～ 6. 876;P <0. 01 ) in smoking patients compared with non-smoking ones. OR was 4. 14 ( 95% CI:2. 157 ～7. 915 ;P <0.01 ) in smoking patients with CC genotype,and was 2. 11 ( 95% CI:0. 442 ～ 10. 08; P > 0. 05 ) in smoking patients with CG genotype compared with non-smoking ones. OR was 0. 29 ( 95% CI:0. 074 ～ 9. 140; P = 0. 082 ) in smoking patients with CG genotype compared with smoking ones with CC genotype,which approximated to statistically significance. OR was 1. 20 ( 95% CI:0. 320 ～4. 505;P > 0.05 ) between non-smoking patients with CC genotype and smoking ones with CG genotype. Conclusion LPL Ser447Ter is a genetic marker for low risk of stroke, and the variation of LPL Ser447Ter can reduce the susceptibility to ischemic stroke in smoking patients.u0000 betw%目的 探讨脂蛋白脂酶(LPL)基因Ser447Ter变异和吸烟的交互作用对缺血性脑卒中发生风险的影响.方法 采用病例-对照设计,对96例缺血性脑卒中患者及117例非心脑血管病者进行研究,采用聚合酶链式反应-限制片段长度多态性(PCR-RFLP)观察LPL基因Ser447Ter变异和吸烟在脑卒中患者中的分布以及二者交互作用对卒中危险性的影响.结果 缺血性脑卒中患者LPLSer447Ter携带比例为10.4%,显著低于对照人群(21.4%,P＜0.05);Ser447Ter携带者发生脑卒中的危险OR值为0.428(95% CI:0.194～0.994;P=0.035);吸烟者较非吸烟者罹患卒中风险OR=3.08(95% CI:2.105～6.876;P＜0.01);CC型中吸烟者发生卒中风险OR=4.14(95% CI:2.157～7.915;P＜0.01);CG型吸烟者与非吸烟者比较OR=2.11(95% CI:0.442～10.08;P＞0.05);吸烟者CG型较CC型发生卒中风险为OR=0.29 (95% CI:0.074～9.140;P=0.082),接近统计学意义;与CC/不吸烟比较,CG/吸烟者卒中风险OR=1.20(95% CI:0.320～4.505;P＞0.05).结论 LPLSer447Ter是脑卒中低危性的基因标记,LPL 基因 Ser447Ter变异减弱了吸烟者对缺血性脑卒中的易感性.
