呼气末二氧化碳分压监测在院前呼吸支持中的应用

摘要

Objective To summarize end -tidal carbon dioxide partial pressure ( PetCO2 ) monitoring application in pre -hospital respiratory support.Methods By monitoring the change of PetCO2 at the end of the numerical and the waveform of different forms, the problems existing in the respiratory support were analyzed in order to rectify.Results In 62 cases of PetCO2 monitoring patients, 34 cases was abnormal, 27 cases returned to normal after timely treatment, the percutaneous oxygen saturation( SpO2 ) at 95% ~99%in 12 cases, 90%~99%in 15 cases;7 patients did not return to normal, SpO2 maintained at 60%~80% in 3 cases, 40%~60% in 1 case, 3 cases below 40%. Conclusion PetCO2 monitoring can not only noninvasive monitoring of ventilation, also can reflect the circulation function and pulmonary blood flow, is commonly used, reliable clinical monitoring means, and gradually applied in emergency medicine and pre-hospital first aid.%目的：总结呼气末二氧化碳分压（ PetCO2）监测在院前呼吸支持中的应用。方法通过监测PetCO2数值的变化和波形的不同形态，分析呼吸支持中存在的问题以便及时纠正。结果62例PetCO2监测患者中34例异常，经及时处理后27例恢复正常，经皮血氧饱和度（ SpO2）维持在95％～99％者12例、90％～95％者15例；7例患者未恢复正常，SpO2维持在60％～80％者3例、40％～60％者1例、40％以下者3例。结论 PetCO2监测不仅可无创监测通气也可反映循环功能和肺血流情况，是临床上常用、可靠的监测手段，并逐渐应用于急诊医学和院前急救。
