Er:YAG 激光联合脱敏牙膏对牙本质小管的封闭效果

         

摘要

目的:评价不同参数的 Er:YAG 激光与脱敏牙膏联合应用对牙本质小管的封闭效果。方法:选择新鲜拔除的完整人第三磨牙6个制备成24个0.8 mm 厚的牙本质片,随机分为4组:A 组(对照)、B 组(单纯 Er:YAG 激光照射,40 mJ),C 组(激光40 mJ 联合脱敏牙膏组),D 组(激光60 mJ 联合脱敏牙膏)。SEM观察牙本质片。结果:SEM显示,A 组牙本质小管口开放,管径一致;B 组牙本质表面部分呈熔融状,部分牙本质小管口被封闭或缩小;C 组牙本质表面呈熔融样均匀改变,牙本质小管口关闭的数量增加;D 组牙本质表面呈熔融样均匀改变,几乎所有的牙本质小管口都呈封闭状态,牙本质表面未见裂隙或碳化等过熔融的表现。结论:适当能量和照射时间的 Er:YAG 激光与脱敏牙膏联合应用较单独使用 Er:YAG 激光能产生更好的牙本质小管封闭效果。%AIM:To evaluate the combined occluding effect of Er:YAG laser and desensitizing dentifrice-with different irradiation parameters on human dentinal tubules.METHODS:6 freshly extracted intact human third molars were prepared into 24 detine slices with the thickness of 0.8 mm,the samples were randomly divided into 4 groups.The samples in group A received no treatment at the controls;in group B were irradiated by Er:YAG laser at 40 mJ,10 pulses/s for 20 s,for 3 circles;in group C were brushed with desensitizing dentifrice,and then irradiated by Er:YAG laser with the same parameters as group B;in group D were brushed with desensitizing dentifrice,and then irradiated by Er:YAG laser at 60 mJ,10 pulses/s for 10 s,for 3 cycles.The dentin slices were then observed under SEM.RESULTS:Under SEM,in group A all dentinal tubule orifices were open and with uniform diameter.In Group B,the dentin surface was uneven,some areas were melted,partial dentinal tubules were occluded or reduced. Group C showed melted uniform changes,the number of open dentinal tubules and diameters reduced.Group Dshowed melted uniform changes,almost all of the dentinal tubule orifices were occluded,no over-therm changes such as crack-ing or carbonization were detected.CONCLUSION:Er:YAG laser with suitalble energy and irritation time combined with desensitizing dentifrice can produce better dentinal tubule orifice occluding effect than the use of Er:YAG laser a-lone.
