Objective: To study guidance curative value of N-terminal pro-brain natriuretic peptide (NT-proBNP) in aged patients with chronic heart failure (CHF). Methods: A total of 280 aged CHF inpatients were selected from our hospital and divided into NT-proBNP group (n= 136, discharged according to plasma concentration of NT-proB-NP) and standard treatment group [n= 144, discharged according to clinical symptoms and left ventricular ejection fraction (LVEF)]. Patients were followed up for one year and cardiac function, rehospitalization rate and mortality rate were observed in two groups. Results: Compared with before treatment, LVEF of both groups significantly improved after treatment; compared with standard treatment group after one-year follow up, there were significant decrease in rehospitalization rate (44.0% vs. 10.3%) and mortality rate (12.5% vs. 4.41%) in NT-proBNP group, P<0. 01 both. Conclusion: Plasma N-terminal pro-brain natriuretic peptide is of important guidance value for treatment in patients with chronic heart failure.%目的:探讨血浆氨基末端脑钠素前体(NT-proBNP)水平对老年慢性心力衰竭的治疗指导价值.方法:选择本院住院的老年心力衰竭患者280例,分为NT-proBNP组(136例,根据测定的血浆NT-proBNP浓度值判断出院),标准治疗组[144例,根据临床症状及心脏彩超左室射血分数(LVEF)测值判断出院],随访一年,观察两组心功能、再住院率及死亡率等预后情况.结果:治疗后,两组LVEF均较治疗前明显改善；随访一年后,与标准治疗组比较,NT-proBNP组根据血浆NT-proBNP水平判断出院的再住院率(44.0％比10.3％)及病死率(12.5％比4.41％)明显降低(P均＜0.01).结论:血浆氨基末端脑钠素前体检测对慢性心力衰竭的指导治疗有重要的临床价值.
