经椎间孔脊柱内镜术后回归社会生活的时期

摘要

目的 通过对患者在经椎间孔内镜术后不同时期回归社会生活的随访分析,对患者恢复正常生活及工作的时期提供参考.方法 回顾性分析2015年9月至2016年5月,68例于我院接受经椎间孔内镜下髓核摘除术患者,其中男42例,女27例,年龄28~53岁,平均38岁.其中7例(10.3%)4周内恢复正常工作及生活,为A组;31例(45.6%)在4~8周内恢复正常工作及生活,为B组;30例(44.1%)在术后8周后恢复日常工作及生活,为C组.随访三组术前、术后1.5个月、3个月、6个月的疼痛视觉模拟评分(visual analogue scale,VAS)评分及Oswestry功能障碍指数(oswestry disability index)评分,利用SPSS 19.0进行统计分析.结果 本组患者术前VAS评分5.78±0.14,术前ODI评分0.70±0.01,术后6个月VAS评分1.53±0.14,术后6个月ODI评分0.13±0.01,与术前比较差异有统计学意义(P<0.05).术后6个月VAS评分A组2.43±0.53,B组1.42±0.85,C组1.20±0.89,A组与B、C两组的术后6个月VAS评分比较,差异有统计学意义(P<0.05);其余各组ODI、VAS评分比较差异无统计学意义(P>0.05).结论 成功实施经椎间孔脊柱内镜手术的患者,4周后恢复正常日常生活及工作更有利于术后的恢复,并维持更好的生活质量及就医感受.%Objective By follow-up analysis, to provide references for the time of social life returning after posterolateral transforaminal endoscopic discectomy. Methods A retrospective analysis was conducted from September 2015 to May 2016. A total of 68 patients ( 42 males and 27 females ) were admitted to our hospital. The mean age was 38 years old ( range: 28 - 53 years ). Group A: 7 cases ( 10.3% ); returned to normal work and life within 4 weeks. Group B: 31 cases ( 45.6% ); returned to normal work and life within 4 - 8 weeks. Group C:30 cases ( 44.1% ); returned to routine work and life 8 weeks later. The VAS score and ODI score of the 3 groups were evaluated preoperatively, 1.5 months, 3 months and 6 months postoperatively. The statistical analysis was performed using SPSS 19.0. Results For all patients: preoperative VAS score 5.78 ± 0.14; preoperative ODI score 0.70 ± 0.01;VAS score 6 months postoperatively 1.53 ± 0.14; ODI score 6 months postoperatively 0.13 ± 0.01; differences were statistically significant ( P < 0.05 ). VAS score 6 months postoperatively: Group A 2.43 ± 0.53; Group B 1.42 ± 0.85;Group C 1.20 ± 0.89. Comparing Group A with B, C, differences were statistically significant in VAS scores 6 months postoperatively ( P < 0.05 ). There were no statistically significant differences in ODI and VAS scores among other groups. Conclusions After posterolateral transforaminal endoscopic discectomy, it is better to return to normal daily life and work 4 weeks postoperatively to facilitate recovery.
