Objective To evaluate the effects of microwave combined with comprehensive psychological interventions therapy on Oral Health-Related Quality of Life before and after treatment of temporomandibular joint disorder (TMD). Methods 42 patients with TMD were diagnosed by RCD/TMD biaxial diagnosis and randomly divided into 2 groups. 21 patients in Control group were treated with pure microwave on joint. Experimental group including the other 21 patients was treated with comprehensive psychological intervention besides the method in control group. Before and after treatment, the assessment of the Oral Health Impact Scale (OHIP-14) was conducted to assess the effect of TMD on the quality of life and mental health of the two treatments. Results Before treatment, OHIP-14 scores 37.85± 9.55 and 36.71 ± 10.2 were treated by the experimental group and the control group, and the differences between the two groups were no statistical significance (P>0.05). After treatment, both OHIP-14 scores improved, but the experimental group scores 15.79±5.82 were lower than the control group 21.43±8.16, and the differences were statistically significant (P<0.05). In the Experimental group, Except for the item 2"taste" of variation of functional limitations field, the item 9 "not very good rest" in psychological disorder field, item 12 "difficult to finish daily work" in social disorder aspect, and item 14 "do nothing" in social disorder aspect, the differences between the scores were no statistical significance;other 10 entries scores were lower than the control group,the score obtained between the difference was statistically significant different (P<0.05). Conclusion TMD has a significant effect on the quality of life,psychology health. Microwave combined with comprehensive psychological interventions, purely microwave treatment all can improve oral health quality of life in patients with TMD, but microwave combined with comprehensive psychological interventions improvement is more apparent.%目的:评估微波联合综合心理治疗对颞下颌关节紊乱病(temporomandibular joint disorder,TMD)患者治疗前后口腔生活质量的影响.方法:通过RCD/TMD双轴诊断确诊为TMD患者42例,随机分成两组,每组21例.对照组给予微波治疗,试验组行微波治疗+综合心理干预治疗.治疗前后进行口腔健康影响程度量表(OHIP-14)的测评,评估微波联合综合心理干预对TMD患者生活质量和心理健康的影响.结果:试验组和对照组治疗前OHIP-14评分37.85±9.55和36.71±10.2,所得评分差异无统计学意义(P>0.05);两组治疗后OHIP-14评分均有改善,但试验组得分15.79±5.82低于对照组21.43±8.16,差异有统计学意义(P<0.05).试验组除功能限制领域中条目2"味觉变差"、心理障碍领域中条目9"不能很好休息"、社交障碍领域中条目12"难以完成日常的工作"以及社交残障领域中条目14"什么事都干不了"所得分值之间差异无统计学意义(P>0.05)外,其它10项条目低于对照组(P<0.05).结论:TMD对患者的生活质量、心理健康存在一定影响,微波联合综合心理干预治疗、单纯微波治疗均可改善TMD患者口腔健康生活质量,但微波联合综合心理治疗效果更为显著.
