Objective To study Influence analysis of the same period of line out the third molar surgery with periodontal treatment for patients of second molar distal periodontal tissue. Methods From February 2015 to January 2016,in our hospital out of mandibular bilateral mesial tilt impacted wisdom teeth in 47 cases,which was randomly divided into the observation group and the control group with each side. The control group was treated with conventional treatment only after tooth extraction, while the observation group was given before and after the extraction of the second molar distal to the periodontal treatment, respectively compared the effect of the two groups. Results There was no significant difference in plaque index (PLI) and gingival index (GI) between the two groups before treatment. After treatment, the observation group PLI (2.02±0.12),GI(1.03±0.15)was significantly lower than that of the control group of (2.78±0.33),(1.68±0.24);There was no significant difference between the two groups before probing depth (PD), and the positive rate of bleeding (BOP).After treatment,the observation group PD (3.45±0.65) mm, BOP (1.45±0.59)%,which were significantly lower than those in the control group (4.37±0.89)mm,(2.11±0.73)%. There was no difference in tumor necrosis factor-α (TNF-α) and C reactive protein (CRP) levels before and after treatment in the two groups, and TNF-α(2.42±0.86)ng/mL, CRP(4.45±0.83) mg/L in the observation group were significantly lower than those in the control group (5.61±1.12)ng/mL,(6.82±1.33)mg/L. The two groups after treatment of alveolar bone crest to the cemento enamel junction distance were significantly lower than that before treatment, the observation group (3.18±0.96) mm was significantly lower than that of the control group (3.55±0.62)mm,the difference was statistically significant (P<0.05). Conclusion The treatment of the third molar extraction can improve the recovery of the distal periodontal tissue of the second molars.%目的:研究第三磨牙拔除手术同期行牙周治疗对患者第二磨牙远中牙周组织的影响并进行分析.方法:选取2015年2月-2016年1月在笔者科室选择拔出下颌双侧近中倾斜阻生智齿患者47例,采用自身对照方法随机分配其一侧为观察组,一侧为对照组.其中对照组仅在拔牙后进行常规处理,观察组则在拔牙前后给予第二磨牙远中面行牙周治疗,分别对比两组疗效.结果:治疗前两组菌斑指数(PLI)、牙龈指数(GI)比较无明显差异;治疗后观察组PLI(2.02±0.12)、GI(1.03±0.15)均显著低于对照组的(2.78±0.33)及(1.68±0.24);治疗前两组牙周探诊深度(PD)、探诊出血(BOP)阳性率比较无显著性差异,治疗后分别观察组PD(3.45±0.65)mm、BOP(1.45±0.59)%均明显低于对照组的(4.37±0.89)mm、(2.11±0.73)%;治疗前两组肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)水平对比无显著性差异,治疗后均有所下降,并且观察组TNF-α(2.42±0.86)ng/mL、CRP水平(4.45±0.83)mg/L均显著低于对照组的(5.61±1.12)ng/mL、(6.82±1.33)mg/L;治疗后,两组牙槽骨嵴顶至釉牙骨质界距离均显著低于治疗前,而观察组(3.18±0.96)mm又显著低于对照组(3.55±0.62)mm,差异有统计学意义(均P<0.05).结论:在第三磨牙拔除手术同期行牙周治疗可有效促进患者第二磨牙远中牙周组织的恢复.
