玻璃纤维桩结合CAD/CAM玻璃陶瓷全瓷冠在前牙美学修复中的临床效果研究

摘要

Objective Through case review, compairing the success rate and complications of aesthetic prosthesis in anterior teeth by glass fiber post and CAD/CAM all glass-ceramic crown or cast metal post and core and pocerlain fused metal (non-noble metal) crown,to evaluate the clinical effect.Methods Collection the patients whose anterior teeth were undergone root canal therapy and restored with post and core and crown more than one year.Dividing the cases into two groups:The treatment group were restored with glass fiber post and composite resin core and CAD/CAM all glass-ceramic crown.The control group were restored with cast metal post and core and pocerlain fused metal (non-noble metal) crown.All restorations were evaluated at 1,6,12 months after restoration in terms of the color match,the integrity of restoration, marginal adaptation, gingival condition and root fracture.Compairing the success rates of two restorations.Results There were 77 patients,the treatment group contained 31 patients,52 teeth.The control group contained 45 patients and 66 teeth.There were 1 case color deviation and 1 case the post and core with the all glass-ceramic crown loose nine months after cementing the crown in the treatment group.4 cases color deviation, 3 cases fracture,1 case marginal adaptation is bad,5 cases appear obviously swollen gums or the gingival discolour, I case root fracture in the control group.Statistic using x2 test to compare the success rate,the results show difference no significant (P＞0.05) at 1 and 6 months.The 12 months survival rate of the treatment group(96.2％) was higher than the control group(78.8％),the results indicated significant differences(P=0.014).Conclusion The effect of the glass fiber post and CAD/CAM all glass-ceramic crown in the aesthetic prosthesis of anterior defect teeth is good.It is worth to be popularized in the clinic.%目的:通过病例回访比较玻璃纤维桩结合CAD/CAM玻璃陶瓷全瓷冠和铸造金属桩核结合烤瓷熔附金属(非贵金属)全冠在前牙美学修复中的临床效果.方法:收集前牙进行根管治疗后桩核加全冠修复时间超过1年的病例并分组,玻璃纤维桩树脂核结合CAD/CAM玻璃陶瓷全瓷冠修复为实验组,铸造金属桩核结合烤瓷熔附金属(非贵金属)全冠修复为对照组,分别于修复后1个月、6个月、1 2个月复查观察两组的颜色匹配、修复体完整性、边缘适合性、牙龈健康状况或牙龈颜色改变以及是否出现根折,比较两组成功率.结果:共收集患者77例,实验组31例,修复患牙52颗;对照组45例,修复患牙66颗.实验组1例颜色不匹配,1例修复后9个月出现纤维桩树脂核与CAD/CAM全瓷冠整体松动;对照组4例颜色不匹配、3例瓷崩、1例边缘不密合、5例牙龈变色或者明显炎症、1例根折.通过x2检验1个月、1～6个月实验组和对照组累计成功率比较(P＞0.05),无统计学差异.12个月累计成功率实验组为96.2％,对照组为78.8％,实验组高于对照组(P=0.014),有统计学意义.结论:玻璃纤维桩结合CAD/CAM玻璃陶瓷全瓷冠在前牙牙体缺损美学修复中效果较好,值得临床推广.
