LAVA序列在膀胱癌临床分期诊断中的应用研究

摘要

Objective:To investigate the sequence of LAVA dynamic contrast enhancement in the diagnostic value of bladder cancer clinical stage. Methods:To collect and select our hospital between January 2013 and May 2013,37 cases of bladder cancer patients,a total of 54 lesions. All subjects were confirmed by our hospital surgery pathology and preoperative both by routine MR scan,diffusion weighted imaging (DWI) scan and LAVA. To evaluate the sensitivity and specificity of LAVA se-quence with routine MRI scan and conventional MRI scan + DWI in the clinical staging diagnosis of bladder cancer patients. Results:In the group of 37 cases of patients with a total of 54 lesions,21 lesions were pathologically confirmed without muscu-lar infiltration,33 lesions with muscular infiltration. Conventional MR + DWI + LAVA correctly diagnosed 31 lesions with mus-cular infiltration and 19 lesions without muscular infiltration,2 false negative of muscular infiltration,and 2 false negative of non muscular infiltration,diagnostic sensitivity and specificity were 93.9% ,90.5% ,respectively,which were significantly higher than that of conventional MR scanning solutions(60.6%,61.9%) and MR scan+ DWI solution(72.7%,71.4%),and the differences were statistically significant (P<0.05). Conclusion:LAVA sequence can effectively improve the detection sensitivity and specificity of patients with bladder cancer muscularis infiltrates,and can provide effective help in the clinical staging and treatment.%目的:探讨肝脏容积超快速采集(LAVA)动态增强扫描序列在膀胱癌临床分期中的应用.方法:收集膀胱癌患者37例,共54个病灶,均经手术病理证实且术前均行常规MRI平扫、DWI及LAVA扫描.评估LAVA序列、常规MRI平扫、常规MRI平扫+DWI在膀胱癌患者临床分期诊断中的敏感性及特异性.结果:37例54个病灶中,病理证实病灶未浸润肌层21个,浸润肌层33个.常规MRI+DWI+LAVA正确诊断病灶浸润肌层31个,未浸润肌层19个,浸润肌层假阴性2个,未浸润肌层假阴性2个,诊断敏感性、特异性分别为93.9%、90.5%,均明显高于常规MRI扫描的60.6%、61.9%及MRI平扫+DWI扫描的72.7%、71.4%,且差异均有统计学意义(均P<0.05).结论:LAVA序列能有效提高膀胱癌患者肌层浸润检出的敏感性及特异性,为临床分期及治疗提供有效帮助.
