Objective To improve the cases of hospital medication errors and nearmiss reported initiatively, and to prevent or reduce the recurrence of similar errors. Methods From July 2016 to March 2017, the reporting process of hospital medication errors and nearmiss were improved by using the analysis tool of PDCA cycle, the initiative concept of employees to report were established, the occurrences of medication errors and nearmiss were analyzed statistically, the improvement measures were formulated for continuous improvement. Results The reported cases of medication errors and nearmiss in our hospital were increasing month by month and stabilized, and com-pared with the previous period, there was a significant improvement in the occurrence of the similar medication errors and nearmiss in our hospital. Conclusion Improving the reported cases of medication errors and nearmiss and formulating targeted improvement measures can significantly improve the safety of hospital medication.%目的 提高医院用药错误和接近失误上报案例的主动性,防止或减少类似错误的再发生.方法 2016年7月至2017年3月,按PDCA循环的分析工具,完善医院用药错误和接近失误的上报程序,树立员工主动上报的观念,并统计分析已发生的用药错误和接近失误,制订改善措施,持续改进.结果 医院用药错误和接近失误的上报例数逐月上升并趋于稳定,医院已发生的用药错误和接近失误后期有明显改善.结论 提高用药错误和接近失误的上报以及针对性改进措施可明显改善医院用药安全问题.
