Objective To promote the rational use of antibacterials during perioperative period. Methods The utilization of antibacterials and rational drug use in 3 kinds of type Ⅰincision operations(thyroid surgery,breast surgery and indirect inguinal hernia)from January to June 2015 and June to January 2016 in a grade three hospital were analyzed retrospectively. Results The use rates of antimicrobial agents were in the standard range during the same period of two years(17. 20%,14. 53%),the rate of non-indicative medication in-creased from 58. 14% to 61. 90%. From January to June 2016,the irrational rate of drug use,the irrational rate of medication timing, and the irrational rate of medication course were 16. 67%,7. 14% and 42. 86%,respectively,which were significantly lower than 39. 53%,30. 23% and 55. 81% in the same period last year. There was no combination medication. Conclusion The usage of periop-erative antimicrobial prophylaxis in the hospital accordance with national requirements,the irrational phenomenon of drug choice,medica-tion timing and medication course has been improved,but they still need to be improved. The hospital should further strengthen the su-pervision of prophylactic use of antibacterials during perioperative period of type Ⅰincisions to promote rational use of antibiotics.%目的 促进围手术期抗菌药物的合理使用.方法 回顾性分析某三级医院2015年1月至6月和2016年1月至6月3种Ⅰ类切口手术(甲状腺手术、乳腺手术、斜疝手术)抗菌药物的使用情况和合理用药指标.结果 抗菌药物使用率两年同期(17.20%,14.53%)均在标准范围内;无指征用药率由58.14%升至61.90%,2016年1月至6月选用药物不合理率、用药时机不合理率、用药疗程不合理率分别为16.67%,7.14%,42.86%,分别低于上年同期的39.53%,30.23%,55.81%,无联合用药情况.结论 该院3种Ⅰ类切口围手术期抗菌药物预防性使用率符合国家的要求,药物选择、用药时机、用药疗程不合理现象有好转,但仍有待改善,建议院方进一步加强对Ⅰ类切口围手术期预防性应用抗菌药物的监管力度,促进合理用药.
